CHECKLIST OF ENTRY REQUIREMENTS:
1. Three (3) hard copies of duly accomplished application form. For Participants or
Entrants of Filipino descent but non-citizens, attach proof of Filipino lineage (may
be NSO-certified birth certificate of relative establishing Filipino lineage).
2. Three (3) clear hard copies (and a soft copy) of the entry’s SEQUENCE TREATMENT.
3. Three (3) clear hard copies (and a soft copy) of the SCREENPLAY (Courier Font, 11
points, with pagination) NOTE: The screenplay must NOT bear the name of the
proponent(s).
4. A brief resume/bio-data with two (2) recent 2x2 photos of the
proponent(s)/entrant(s) (apart from the photos attached/included in the
application form).
5. Samples of previous film/video work, if any, on DVD or data file format (stored in
a flash drive) properly labeled with the proponent(s)’ name(s), project title and
contact details (e-mail, phone, home and/or office address).
6. All entries must be submitted in a long brown envelope properly labeled with the
proponent(s)’ name(s), project title and contact details (e-mail, phone, home
and/or office address), plus the category.
7. All these materials must be sent via certified mail or dropped off no later than 7:00
pm, on 30 APRIL 2015 at the CineFilipino Film Festival™ Office c/o Unitel
Productions, Inc. – 4th Floor Salustiana D. Ty Tower, 104 Paseo de Roxas corner
