Traveler Agreement Form
This form is to be completed by each traveler; student, chaperone, or director.
I will be attending the Fine Arts Trip to Orlando, Florida on Wednesday, February 10-Saturday, February 13, 2016.
By signing this form, I am responsible for adhering to the guidelines and the payment schedule as presented herein.
1.
Behavior – I understand that this trip is considered to be an extension of the school day. All county and school rules apply.
Student Travelers will also receive a separate Behavior Contract containing Trip Rules, Procedures, and Expectations.
Any noncompliance of the rules will result in a conference with the director(s) and appropriate disciplinary measures.
2.
Payment Schedule – I understand that payments must be made on time or the traveler may forfeit their right to attend.
The total cost of the trip, based on a room of 4 students or 2 chaperones, is $520.00 per person and includes:
Round-trip motor coach transportation, 2 nights-hotel accommodations with security guards,
Admissions to Universal Studios, Islands of Adventure, the OrlandoFest Workshops,
3 breakfasts, 3 lunches, 3 dinners including the Pirate’s Dinner Adventure, a tour escort, and a trip hoodie.
Payment 1 - $175.00 (+ $25.00 OPTIONAL Cancellation Insurance*) due on/before 9/18/2015
THIS COMPLETED TRAVELER AGREEMENT FORM IS DUE WITH PAYMENT 1 ON/BEFORE 9/18/2015
Payment 2 - $175.00 due on/before 10/30/2015
Payment 3 - $175.00 due on/before 12/4/2015
Payments may be in the form of cash, money order or check payable to WMS.
Checks must include an address, phone number, and the student’s name. A single check may not exceed $300.00.

Refund Policy - *OPTIONAL CANCELLATION INSURANCE
OPTIONAL: An additional $25.00 is due with Payment 1 on/before 9/18/2015. This is not required, it is optional.
If a traveler cancels for sickness, injury, or some unforeseen tragedy, 100% of the payments to date will be refunded.
If a traveler cancels for any other reason up to 48 hours prior to departure, 75% of the payments to date will be refunded.
Trip Payments are ONLY refundable to travelers who purchase the optional cancellation insurance.
