Position Summary:
Provide discipline-specific treatment for pediatric population with special needs, including evaluation,
planning, documentation, and professional communication. Also, provides discipline-specific
consultation, education and supervision to parents, caregivers, team members, and community
partners as appropriate.

Essential Job Duties & Expected Outcomes:
• Understands and demonstrates support for ESGW’s mission, vision, values, culture,

• Maintains complete confidentiality of information and images involved with
program/service participants, staff and/or other fiscal matters, including HIPAA

• Maintains regular and predictable attendance, adheres to ESGW attendance policy,

• Adequately prepare and ensure timeliness for all patient interactions including but
not limited to patient visits and family coaching.

• Ensures data entry and documentation is compliant with payer source regulations
and ESGW policy.
• Complies with federal and state practice requirements, HIPAA guidelines, CARF (if
applicable), and ESGW requirements.
Quality &
• Follows regular checkpoints and performance indicators to consistently meet

develop goals to ensuring that treatment plans are appropriate.
Implement a family centered approach that guides the development of treatment
plans.
Provide coaching to family members and/or clients that incorporate routine based
intervention strategies.
Complete participant’s re-certification every (6) six months and re-evaluate
annually.
Monitor episodes of care and treatment strategies to reach desired patient
outcomes.
Ensure that billable hours are supported by appropriate documentation and are
reported for third party reimbursement in a timely and thorough manner
Maintain a designated caseload, caseload size determined by FTE.
Adhere to current APTA policies, standards, codes, and guidelines as need to
provide necessary and high quality services.
Engages in interagency and staff meetings, trainings, and program planning
sessions.
Applies effective written and oral communication skills with co-workers,
participants, and public.
Seeks direction from supervisor when needed.
Utilize best practice methodology in the evaluation, assessment, development of
measurable objectives and goals, and development of the plan of care that reflects
medical necessity.
Ensure services provided are guided by appropriate evidence based practices and
consistent strategies are being used to achieve identified goals.
Participate in professional development courses and/or training that supports an
understanding of current best practice methodology applicable to designated
caseload
Demonstrates reliability, communicates constructively, listens actively, and shares
willingly with the team.

Essential Job Requirements:
Education
• Bachelor's degree
• Minimum of a Bachelor’s or Master’s Degree and current licensure as an Physical
Therapist in the State of Montana.
Experience
• 0-2 years
• Minimum of (12) twelve months of professional experience in a pediatric clinic
setting, preferred but not required.
Requirements
• Proficient oral and written communication skills.
• Excellent time management and organizational skills.
