FINAL EVALUATION 6. FEEDBACK FORM 7. CONCLUSION INTRODUCTION We all are very familiar with the pharmacy word. Every pharmacy is a part of our community. I finished my degree pharmacy from India. The actual practice of pharmacy in India is totally different from Canada. When I came in Canada and enrolled in retail pharmacy assistant course, I feel that all the learning objectives and scope of practice is totally different from India. In Canada and especially in Ontario government provides lot of drug benefit and trying to fulfill all the necessary requirements for every people. Every health care provider recognizes the patient problems and solves at their best level and also provides safety to patients. Field placement course is not only part of my course but it is actually opportunity to apply my knowledge which I learned during this course in actual pharmacy practice. These work placements give me a chance to perform my all duties as a retail pharmacy assistant so it would be helpful for me in my pharmacy career. DESCRIPTION OF PHARMACY. My placement was in Victoria medical pharmacy. It is a methadone pharmacy. It is a independent pharmacy. This pharmacy is located in downtown, rough and slum area of Niagara Falls. There are four members work in pharmacy like two pharmacists, two pharmacy technicians or assistant. This pharmacy opens at 8.30 am to 7pm from Monday to Friday, at 9 am to 4 pm on Saturday and from 10am to 1 pm on Sunday. Working hours are allocated in such a way that there are enough staff to complete daily activities. This is a methadone pharmacy so flow of patients is not too much like other pharmacy. Patients frequently come for their methadone drinks on same time. The average number of prescription runs is 50-200 in ranges. The average proportion of repeat to new prescriptions is 60/40.This is not very large pharmacy. In this pharmacy, there is a small, separate window for methadone patients and at the cash counter patients pickups their medication on the front. DESCRIPTION OF PHARMACY SERVICES. There are so many professional and dispensary services offered by this site are blister packing, consultation, and compounding of methadone and stock solution. Methadone therapy is used for the treatment of opioid dependence. Pharmacists in an accredited pharmacy are permitted to dispense methadone in individually labeled and fully diluted daily doses for the treatment of opioid dependence pursuant to a written prescription from an exempted physician. The pharmacist and physician play an important and complimentary role in the interdisciplinary model of methadone maintenance treatment. The role of the pharmacist is to optimize the patient’s drug therapy and establish a therapeutic pharmacist-patient relationship with all patients for whom s/he dispenses drugs. The pharmacist must be responsive to prescribed changes in the methadone treatment and able to provide methadone in a timely manner. Pharmacists are responsible for the safety and integrity of the drug until such time as they have dispensed directly to the patient or transferred its custody to an exempted physician or his/her delegate. When the provision of methadone has been entrusted to a delegate, the accountability and responsibility for the administration of methadone doses continues to rest with the physician. The designated manager (DM) and all pharmacy staff employ the same respectful, professional approaches and attitudes towards MMT patients as they would toward any other patient of the pharmacy. Documentation: The observation of methadone ingestion is documented daily in the patient record so that it can be determined where each dose of methadone went, for both patient care and auditing purposes. Documentation of methadone ingestion must include the patient’s name daily dose, date, time and place where the administration was observed. Maintaining patient confidentiality and privacy: Patient administration of methadone in a pharmacy must be done in an area and manner which ensures patient confidentiality and privacy. This pharmacy helping the addicted person to get off drugs and provides all required pharmaceutical care. The ideal customer service should involve patient get their medication on time, get satisfied response of their concerns. Description of dispensary and dispensing systems: In this pharmacy, there are different racks for all dosage forms like creams, ointments, some therapeutic devices, over the counter drugs, prescription drugs. In this pharmacy nexxsys software is used. For new prescription when we enter the patient’s profile then click on ‘new’. Then enter new drugs and fill in and finally third party payment and adjudication is done. Then pharmacist checks all the process. If there is any repeat on it then first we have to open patient’s profile and if repeats are remaining then only click on refill and adjudicate online for third party payment. Nexxsys computer system simple but complicated. We can enter directly the packsize. Pharmacy technician process the prescription and fill it on computer. Technician also checks the inventory, order the drugs, receives the phone calls, handle the cash register, counting the medication, assist the pharmacist. Pharmacy technician also doing billing for drugs and accounting all the medication. Three example of online responses: 1. prescriber ID error-use MH intervention code to override 2. Identical claim processed (when billing carries)-use UA intervention code. 3. Drug interaction potential- consult with pharmacist and if ok use UG intervention code. Controlled drugs are stored in the shelf like codeine, testosterone, methylphenidate, hydromorph etc. Some pharmacies store their narcotics in a locked cupboard or safe. Other pharmacies store these drugs unlocked, on dispensary shelves interspersed with other prescription stock bottles. Twice annually, all narcotics, controlled drugs need to be counted and a record kept of all stock. From my experience of placement in methadone pharmacy, I can say that I learned as much as I can do. My placement was just like opportunity for me to move towards my pharmacy career. I also enjoyed at that place.
