Continuing Professional Development Audit Form - Instructions and Guidance
The 2014/15 Continuing Professional Development year runs from 1 July 2014 to 30 June 2015. As part of the 2014/15 audit of your CPD
activity, you must complete this form or the version available to download at www.rad.org.uk/cpdaudit and return it with supporting evidence.
We will not be able to complete the audit of your activity without this form:
Completing Section 1
Select either:
Option 1; return this form with accompanying evidence to CPD department.
Option 2 return this form and share your evidence with the Academy via your profile on the ePortfolio.
Please contact the CPD department at cpd@rad.org.uk for further information on how to share your evidence via the ePortfolio.
For both options you must evidence that you have met the minimum requirement of 24 hours of CPD or your remaining requirement if you
received a part-exemption.
For each activity that you have participated in, please include a copy of one piece of evidence only. Please note, we do not need evidence of
more than 24 hours of CPD.
All evidence must show:
1. Your name
2. The date (or range of dates) of the activities
3. Title of the activities
4. Confirmation of your attendance/participation in the activities
Any evidence you include with the Audit Form must clearly verify your attendance/participation in the activity.
Examples of evidence include but are not restricted to:
• copies of certificates
• letters of attendance
Independent CPD:
A maximum of 8 hours may be completed by participating in Independent CPD; each Independent CPD activity must be no more than 2 hours.
As it is difficult to evidence Independent CPD, we do not require you to provide evidence of this. If however, for any reason we require
clarification on a specified Independent CPD activity we reserve the right to ask you to provide further information.
Please do not send original documents, as we cannot return them to you unless you provide a self- addressed stamped
envelope.
Completing Section 2
You must sign and date the form to confirm the information you have given is true and accurate to the best of your knowledge. Submission of
a false form is a breach of the Code of Professional Practice for teachers registered with the Royal Academy of Dance.
What happens when we receive your form?

We will send you an email confirming receipt of your audit data.

We will review the information you have sent us.

If we need any further clarification or information regarding your evidence we will contact you either via email or telephone.

Once we have reviewed the information you have sent us we will contact you to confirm whether you have successfully completed
your CPD requirement for the 2014/15 CPD year.

If after further consultation with you, you have been unable to demonstrate that you have met the minimum CPD requirement we
will send confirmation that you have not completed your CPD requirement for the 2014/15 CPD year and explain what will happen
next.
