Prepared By: Laura Davenport Page 1 2/9/2016 University of Washington Group 1 Safety Committee Minutes Approval of the Minutes: Minutes of August, September and October are approved with correction to the spelling of Dian’s name on September minutes. U-Wide Meeting Highlights: Elections were discussed. A new tool kit for labs was discussed. The toolkit is completely free. There’s also a new lab safety video out. Please see the EH&S website for more details. Incident Report Backs: No discussion of report backs at this meeting. Current Incident Report Reviews: The committee reviewed the September accident and incident reports. Incident 2011-10-017: Ctr for Commercialization off campus picnic incident. While employee played catch football, he managed to catch the ball wrong and injured his little finger. Resolution: Employee sought comprehensive medical assistance. No further action necessary. The committee accepted this resolution. Incident 2011-10-029: Benefits Office. Employee twisted ankle and fell. No apparent reason for the incident. Resolution: No action required for this fluke incident. The committee accepted this resolution. Incident 2011-10-031: Office of Minority Affairs. Employee cut finger with pair of scissors. Resolution: Pay careful attention when using scissors. The committee accepted this resolution. Incident 2011-10-049: Applied Physics Lab. Employee hurt back while reaching under a bench for shoes. Resolution: Employee placed on restricted work for the time. The committee accepted this resolution. Employee fell from heat register while trying to close the curtains per UW Tower 16th floor procedure. Curtain closure devise in awkward position to work with. Resolution: Identify and implement changes to allow safe access. Staff all advised to avoid walking on window sills. Connie will follow up with Tower Facilities to discuss better window treatment closures. Incident 2011-10-085: Library conference at Pack Forest Building. A conference attendee was walking in front yard of building when something flew into her eye. 911 contacted and evaluated. Another conference attendee provided Benedryl for what appeared to be an allergic reaction. No supervisory comments. Leslie will inform the College of the Environment Group know about the incident. Incident 2011-10-089: Classroom Support Services. Improperly stored metal table fell on employee’s foot. First aid provided. Table left where it fell. No supervisory comments. Leslie will follow up with supervisor. Actions Pending: Incident 2011-10-050: Connie will follow up with Tower Facilities to discuss better window treatment closures. Incident 2011-10-085: Leslie will inform the College of the Environment Group know about the incident. Incident 2011-10-089: Leslie will follow up with supervisor. Good of the Order: Mary provided a handout of the nominees for the committee. Discussion about the election process and a balance of representation ensued. Because election methods are decided by each committee, Group 1 attendees decided the following: Every employee represented by Group 1 will be entitled to cast one vote for their departmental representative. The top 2 candidates will be chosen for the elected position as outlined here: o For units with under 200 employees, there will be one representative and 1 alternate. Voting authority will be limited to one vote even if both are present.
