Tantisira Beauty Rewards
Terms and Conditions
It is each participant’s responsibility to read and understand the terms and conditions
regarding Tantisira Beauty Rewards program.
Registration for the Tantisira Beauty Rewards program is required to accumulate
Rewards.
Rewards will NOT be added retroactively to any account.
Tantisira Beauty Rewards earned by a member cannot be shared or transferred to other
members’ accounts.
Gift certificates purchased by the participant for their own use will not accumulate
Tantisira Beauty Rewards.
When using rewards to purchase treatment, or series of treatment or skincare products,
you do not earn additional Rewards for those services or products rendered.
* Aesthetic procedures eligible to earn rewards are injections of muscle relaxants (Botox,
Dysport), dermal fillers (Restylane, Juvederm, Perlane, Belotero, Radiesse), Fractional
CO2 Laser Treatment, Accent XL Body Contouring and Varicose Vein Therapy.
**Skincare products eligible to earn Rewards are True Vision Center Hawaii skincare
line, Latisse, Color Science Pro Sun Forgettable Mineral Sun Protection and Triple
Whitening Cream.
***New patients are defined as clients who have never received aesthetic procedures
from Melanie Tantisira MD or True Vision Center Hawaii. Ophthalmology patients of
Melanie Tantisira MD and True Vision Center Hawaii are eligible to become participants
and considered “new” patients.
Ophthalmology procedures and co-pay amounts are not eligible for Tantisira Beauty
Rewards.
The participant’s account or accounts must be in good standing to receive or to redeem
Tantisira Beauty Rewards.
Inquiries or disputes regarding Tantisira Beauty Rewards to be reimbursed for an
aesthetic procedure or skincare product (s) must be submitted in writing.

If an appointment is cancelled or rescheduled, you must give a minimum of 24 business
(Monday – Friday) hours before the appointment. Any participants who fail to comply
with this policy are subject to a cancellation of their Rewards and all Rewards. Any
participants who fail to show up for a scheduled appointment without proper notification
will automatically forfeit all Rewards.
Melanie Tantisira, MD, LLC, Melanie Tantisira, MD (MTMD), reserves the right to
terminate Tantisira Beauty Rewards or to change these Terms and Conditions,
regulations, or benefits of participation, in whole or in part, at any time with or without
notice, even though changes may affect the number of Rewards already accumulated.
MTMD may also withdraw, limit, modify or cancel the number of rewards required for
reward redemption or change the number or type of rewards offered. MTMD will have
final decision should any disputes over Tantisira Beauty Rewards or the awarding of
Tantisira Beauty Rewards arise.
MTMD reserves the right to terminate a member’s account and cancel accumulated
Rewards, if MTMD believes, in its sole discretion, that the member is abusing the
program, or is misrepresenting any information furnished to MTMD.
Any Tantisira Beauty Rewards member who fails at any time to earn rewards for a period
of twenty-four (24) months is subject to termination of his or her membership and
forfeiture of all accrued Rewards.
Should MTMD offer rewards from any vendor, MTMD will not be responsible for any
vendor’s participation in or withdrawal from the Tantisira Beauty Rewards.
