Methods: Child development centers in Lablae district, Uttaradit province were randomized in two groups.Comprising of 4 centers. Two child development centers, in study group, were offered comprehensive caries preventive program and other two centers were in control group. All groups were offered oral health education and practical tooth brushing training for teachers and parents, tooth brushing with fluoride toothpaste after lunch meal and applications of fluoride varnish every 6 months. Two additional preventive measures (sealant applications with FUJI VII and partial caries removal with FUJI IX GP EXTRA) were provided according to individual needs in only study group. Two hundred and twenty three 25-38-month-old participants who had dental caries were examined according to ICDAS II criteria and debris index (DI-S) at based line and after 6 months. The mean of dmfs/dmft increment and DI-S were used to compare caries preventive effect and oral health status by ttest statistical analysis. Result: There were 197 participant in this study after six month. The means of dmfs increment in study group (n=103)was 1.14±2.87 and control group (n=94) was 3.34±5.19. The dmfs increment in study group was significant lower than control group( p=0.009).The means of dmft increment was 0 . 3 7 ± 1 . 0 8 and control group was 1.64± 1.92. The dmft increment in study group was significantly lower than control group (p<0.001). The means of debris index (DI-S) of study group was 1.51±0.55 and control group was 1.62±0.61. There was no significant difference between two groups (p>0.05) conclusion: Comprehensive caries preventive program that consist of oral health education , tooth brushing after lunch meal with fluoride tooth paste , applications of fluoride varnish , sealant and partial caries removal with GIC in child development center were highly effective in caries prevention than control group.
