COMMUNITY HOSPITAL JOB DESCRIPTION
Registered Nurse
MAIN FUNCTION:
Provides nursing care to patients in an efficient, professional, compassionate and caring manner.
Directs the activities of paraprofessional as they provide care. Responsible for assuring effective
communication with patients, support persons and members of the healthcare team. Responsible for
implementing nursing care of assigned patients via the Nursing Process including assessment,
diagnosis, planning, implementation and evaluation. Acts in compliance with hospital, Nursing
Department policies/procedures and the Nurse Practice Act of the State of Ohio. Follows CH Nursing
Philosophy and practice model which are derived from the Synergy model and appropriate Service
Standards as outlined in Employee Handbook.
RESPONSIBLE TO: Department Manager, Assistant Manager, Charge Nurses
MUST HAVE REQUIREMENTS:

Graduate of NLNAC or CCNE School of Nursing, and licensed in Ohio

BLS certification must be maintained while in this position

Unit Specific Requirements:
AC / PAT / PACU RN
MAIN FUNCTION:
To assess, evaluate, implement, collect data, document, monitor, administer pre and post operative
medications to all surgical patients. Directs the care delivered by others in AC/PAT/PACU. Assist
with physicians with procedures. Prioritize care for all types of surgical patients.
MUST HAVE REQUIREMENTS:

ACLS within 1 year of hire, and maintain while in position
PREFERRED ATTRIBUTES:

Previous perioperative nursing experience

Ability to perform phlebotomy, and IV therapy

Recognize cardiac arrhythmias and institute appropriate interventions
OTHER REQUIREMENTS:

Required to take "scheduled on call" status (PACU)


Cardiac Cath/Special Procedure Lab
MAIN FUNCTION:
Also, functions independently in the Cath Lab/Special Procedures holding room and provides nursing
care in a calm, efficient manner. Admits and discharges patients. Provides pre-procedure teaching and
assures physician orders are done prior to procedure. Recovers post-procedure patients according to
dept policy and accurately follows physician post-procedure orders. Assists in transferring specific
patients to other hospitals directly from the holding room. Provides follow-up care by phoning patients
next day. Provides nursing support in other areas if needed (CT for biopsies).
Keeps patients safe & calm while assisting physicians in cardiac cath/special procedures. Functions
independently in the lab while using professional judgment in performance of services. Accurately &
promptly implements physician's orders. Scrubs, circulates & records all cases done in the lab &
helps to recover patients. Acts in compliance with hospital policies/procedures. Provides care to
patients in a calm, efficient manner.

Cardiac Rehab Nurse
MAIN FUNCTION:
Collaborates with the exercise physiologist to prescribe and monitor exercise workloads and target
heart rates appropriate to each individual during each exercise session. Monitors and records vital
signs during exercise sessions. Communicates effectively with the healthcare team to monitor
compliance and appropriately manage symptoms. Develops teaching curriculum in conjunction with
Nutrition Services, Sleep Clinic, and the exercise physiologist including lectures, videos, class
participation and patient educational materials.
MUST HAVE REQUIREMENTS:

ACLS within 1 year of hire, and maintain while in position

Ability to read and operate cardiac monitor, recognize normal/abnormal rhythms, and institute
appropriate interventions

Demonstrate knowledge and use of Vasoactive medications
OTHER REQUIREMENTS:

Prescribes initial target heart rates and exercise with in conjunction with exercise physiologist

Ensures appropriate warm-up, exercise workloads, and cool down for each client

Adjusts workloads according to observed or reported symptoms, or as requested by the exercise
monitor

Develops cardiac rehab teaching curriculum and implements lectures and materials and reviews /
revises them every three months
Cardiovascular Services Registered Nurse
MAIN FUNCTION:
Provides nursing care to inpatients and outpatients in the Cardiovascular Services Department by
working within the nursing process in an efficient, professional, compassionate and caring manner.
Directs the activities of paraprofessionals as they provide care within the department. Communicates
effectively with patients, support person(s) and other members of the health care team. Acts in
compliance with hospital Nursing Department policies/procedures and the Nurse Practice Act of the
State of Ohio.
MUST HAVE REQUIREMENTS:

Critical Care Transport Registered Nurse
MAIN FUNCTION:
Assesses, plans, implements, evaluates and documents the care of emergently ill medical and
surgical adult patients during transport to other facilities. Supports and maintains the physiologic
stability of patients. Prioritizes information to take immediate and decisive action. Responds with
confidence and adapts to rapidly changing patient conditions. Respond to the unique needs of
patients and families coping with unanticipated treatment, quality of life and end of life decisions.
Responsible for assuring effective communication with patients, support persons and members of the
healthcare team. Acts in compliance with hospital, Nursing Department policies/procedures, the
Nurse Practice Act of the State of Ohio and appropriate Service Standards as outlined in Employee
Handbook.
RESPONSIBLE TO:

MUST HAVE REQUIREMENTS:

Endoscopy RN
MAIN FUNCTION:
To assess, evaluate, implement, and monitor the care of patients undergoing GI procedures and those
patients who are receiving Medical Outpatient Services. To assist physicians with procedures and
monitor patients who are receiving moderate sedation. Prioritize the care of all patient populations.
MUST HAVE REQUIREMENTS:

1 years related medical surgical experience
PREFERRED ATTRIBUTES:

ACLS

Previous endoscopy experience
OTHER REQUIREMENTS:

Must be able to respond to the hospital within 30 minutes

Must be able to work flexible hours including half days

Cleans and checks scopes and instruments according to procedure

Emergency Dept Staff RN
MAIN FUNCTION:
Assesses, plans, implements, evaluates and documents the care of emergently ill medical and
surgical patients of all ages. Assists with special procedures. Prioritizes the care of critically ill and
less acutely ill patients. Directs the care delivered by others in the ED.
PREFERRED ATTRIBUTES:

CEN (Certified Emergency Nurse)

TNCC ( completion of Trauma Nurse Core Curriculum)

Current general nursing, emergency or critical care experience. Completion of emergency or
critical care course.

Minimum of 1 year current experience in hospital or clinical setting as an RN

Float Registered Nurse- ED, ICU, PCU, InPt Rehab
MAIN FUNCTION:
Float staff will be assigned to one unit manager for performance evaluation, but will also be
responsible to management team where they are assigned each shift. Float staff are held accountable
to the job description for their base units (ICU, PCU, ED, InPt Rehab).
MUST HAVE REQUIREMENTS:

ACLS within 1 year of hire, and maintain while in position

Ability to read and operate cardiac monitor, recognize normal/abnormal rhythms, and institute
appropriate interventions

One year current hospital nursing as RN

Must attend a combined total of 75% of staff meetings for units assigned

Home Health Registered Nurse
MAIN FUNCTION:
Provides nursing care in patients’ homes on a skilled, intermittent basis. Coordinates patient care
with other disciplines, PT,OT, Home Health Aide, Speech Therapy and Social Worker to ensure that
patient meets their goals. Follows Medicare Condition of Participation requirements in delivering
patient care.
MUST HAVE ATTRIBUTES:

One year hospital nursing experience or long term care experience

Hold a valid driver’s license

Home Health Services RN Call Nurse
MAIN FUNCTION:
Provides direct on-call coverage to home health patients under the direction and coordinator of the
Clinical Coordinators. Answers all phone calls and makes decisions according to the patient’s best
interest. May need to make patient visits depending on patient’s situation while on-call. Reports calls
to Clinical Coordinator for follow-up.
Inpatient Rehab RN
MAIN FUNCTION:
Assesses, plans, implements, evaluates and documents care of the Rehabilitation patients. Uses the
Functional Instrument of Measure to develop goals for improving functional status.

MAIN FUNCTION:

adolescent/adult patients. Provides acute stroke care per protocols of the WCH designated stroke
unit. Assists with special procedures. Directs the care delivered by others in the department.
Collaborates with Physicians to evaluate patient readiness for transfer out of critical care
environment. Accepts med surg and telemetry overflow patients as necessary. Recovers MAC local
patients when PACU not open.
MUST HAVE REQUIREMENTS:

ACLS within 1 year of hire, and maintain while in position

1 year current hospital nursing; New graduates only with completion of critical care preceptorship
or current Nurse tech position

Complete NIH Stroke Certification with 6 months and maintain while in position
Other Position Expectations
a.
Ability to interpret and operate cardiac and other invasive and noninvasive monitors, recognize
normal/abnormal rhythms /trends, and institute appropriate interventions

Maintain competency in:

Caring for patients with complex needs including hemodynamically compromised, acute
stroke and mechanically ventilated patients

Safe use of less invasive, life supporting modalities i.e BiPAP, ETCO2, arterial pressure
based CO/CI & SVO2 monitor, core temperature monitors

Progressive Care Registered Nurse
MAIN FUNCTION:

adolescent/adult patients with a cardiac focus. Assists with special procedures. Directs the care
delivered by others in the department. Designated stroke unit. Accepts med surg overflow patients
as necessary. Recovers cardiac cath/special procedure patients. Provides care for patients on
intravenous vasoactive medications (step down patients).
MUST HAVE REQUIREMENTS:

Cross train and maintain competency to Step-down Nursing within 1 year


SANE (Sexual Assault Nurse Examiner)
MAIN FUNCTION:
Provides timely and accurate collection of forensic evidence in sexual assault cases. Collaborates
with physicians, local law enforcement, county prosecutors and survivor advocate to maintain chain of
evidence.
MUST HAVE REQUIREMENTS:

Graduate of NLNAC or CCNE School of Nursing, and licensed in Ohio

1 year current experience as a RN

Availability for on-call rotation including weekend and holiday coverage with a maximum response
time of 60 minutes
PREFERRED ATTRIBUTES:

Successful completion- of SART (Sexual Assault Response Team) training- mandatory within 1

Surgery RN
MAIN FUNCTION:

Responsible for circulating and scrubbing on surgical procedures, which includes caring for and
meeting the surgical patient's needs
MUST HAVE REQUIREMENTS:

2 years current hospital clinical nursing experience

Ability to respond to the hospital in 30 minutes when assigned


MAIN FUNCTION:
Under supervision and direction of the operating surgeon, functions during the assistant role until
conclusion of the operative procedure. Does not concurrently function as scrub nurse. When not
working as a RNFA, works as scrub or circulating nurse as directed.
MUST HAVE REQUIREMENTS:
 Current CNOR certification, completion of an RNFA program, certification by the National Institute
of First Assistants (NIFA) as a RNFA

3 years diversified perioperative nursing experience both scrubbing and circulating proficiency
OTHER REQUIREMENTS:
a. Able to respond to the hospital in 30 minutes when assigned
b. Able to work flexible hours

Maintains RNFA status
d. Applies principles of asepsis technique according to standards. Follows infection control
according to current guidelines

Recognizes safety hazards and initiates appropriate corrective action

Applies knowledge of surgical anatomy, physiology, and operative technique relative to operative
procedure wherein RNFA assists

Performs positioning, prepping, and draping of patient

Provides hemostasis by clamping blood vessels, coagulating bleeding points, ligating vessels,
and by other means as directed by the surgeon

Provides exposure through appropriate use of instruments, retractors, suctioning, and sponging
techniques

Handles tissue as directed by the surgeon during the operative procedure

Performs wound closure as directed by the surgeon, sutures the peritoneum, fascia,
subcutaneous tissue and skin

Applies surgical dressings

Assists with transferring the patient from the operating room

Practices within limitations of preparation and experience

Maintains continuing education relative to practice

Verifies correct sponge, tape, needle and instrument count with other nursing personnel in room

Labels specimens properly and takes to area for transfer to Lab
Women’s Pavilion
MAIN FUNCTION:
Assesses, plans, implements, evaluates, prioritizes and documents the care of the obstetric patients.
Assists with labor, surgical procedures, mother/couplet care, newborns, and triage of the obstetric
patient. Communicates the progress of the obstetric patient with the physician on regular basis.
Initiates, interprets, and documents EFM (Electronic Fetal Monitoring) according to AWHONN
(American Women’s Health Obstetrics and Neonatal Nursing) and the ACOG (American College of
Gynecology) of patients. Prioritizes care of the newborn according to the American Association of
Pediatric guidelines.
MUST HAVE REQUIREMENTS:





Neonatal resuscitation required within 6 months of hire and must be maintained
Demonstrate knowledge in caring for patients in post anesthesia recovery
Completion of OB Recovery competency within 6 months of hire
Initiate, interpret, and document EFM (Electronic Fetal Monitoring) according to AWHON
(American Women’s Health Obstetrics and Neonatal Nursing) and ACOG (American College of
Gynecology) within 6 months of hire
Primary Nursery Nurse- attend STABLE program within 1 year, and demonstrate knowledge of
elevated nursery care

Wound Center RN/Case Manager
MAIN FUNCTION:
Under supervision of Clinical Manager, performs primary function of RN in assessing, planning,
implementing and evaluating care of patient in the Wound Center. Administers hyperbaric oxygen
therapy and diagnostic testing of patients under the supervision and orders of a hyperbaric physician
RESPONSIBLE TO:
Program Director, Clinical Manager, RDC
MUST HAVE REQUIREMENTS:

2 years medical/ surgical experience

Completion of Principles of Hyperbaric Medicine and Wound Healing Course
PREFERRED ATTRIBUTES

Wound care experience

Wound care certification
OTHER EXPECTATIONS:

Assess, stage, measure and the photo’s of wounds accurately and in accordance to policy

Assist physicians with wound care

Implements wound care and HBO protocols when clinically indicated

Accurately completes the superbill and ensures that all orders and charges are initiated
accurately

Coordinates activities with other disciplines (rehab, nutrition, diabetic education, home health,
DME, nursing homes and other ancillary services)

Completes the 4 week reviews in a timely manner and initiates all recommendations to the plan

Fosters continuity of care between WHC and care taker (home, hospital, HH, etc)

Prepares for clinic to enable accurate and complete updates to the physician

Triages telephone calls and assists with scheduling
