Start Time is 5:45 PM – Championship is 9:00 PM
(Registration from 5:00-5:30 PM – Rules Meeting at 5:35 PM)

Cost per Player: $20
Alumni are welcome to create their own team or can be placed on a team. Tournament will be
played 7 on 7. Each team needs a minimum of 9 and can have a maximum of 12 players. A
minimum of 4 Buffalo Football Alumni must be on the field at all times. Non-Alumni are welcome
to join the team as long as this rule is followed! Each individual must return this form signed and
paid for prior to the deadline.

PM. I realize that there is a possibility of injury because of the aggressive nature of the game and
will not hold USD 457, Buffalo Football, or those associated with the Buffalo Football Program
liable for any injuries sustained as a result of participating in this game. All injury related
expenses shall be the responsibility of the participant.
