This form can be mailed to Keswick Creamery, 114 Lesher Rd, Newburg PA 17240 or emailed to
cheese@keswickcreamery.com by May 1st. If you send this form by email please send a check to the address
above by May 1st. Sign up before April 15th and receive $10 in Keswick coupons to use at the Carlisle
farmers market.

PAYMENT OPTIONS
Full Amount:
$240 for a Regular Share (12 biweekly pickups of ¾ pound of goat + 1 pound cow
cheese = 21 pounds of cheese)
$120 for a Small Share (12 biweekly pickups of ¼ pound of goat + ½ pound cow
cheese = 9 pounds of cheese)
Payment Plan:
You will pay 25% by May 1st and three additional payments on June 1, July 1 and August 1. We
will send reminder emails about the additional payments.
