Note: For more information about how to undertake a habitat quality analysis please refer to the Guide to determining terrestrial habitat quality,
which is available on the Queensland government website and can be accessed here.
For all environmental offset applications you must:
 Complete form (Environmental Offsets Delivery Form 1–Notice of Election and Advanced Offset Details)
 Complete any other forms relevant to your application
 Provide the supporting information identified on the forms as being required to accompany your application.
This form is necessary for species and ecosystem land-based offsets. Please note that for a staged offset, this will
form will be required for each stage that is being delivered as a land-based offset.
