The Health Center would like to help you meet your Wellness Goals this summer!

How does this program work?
Make an appointment to meet with Deborah or Lee.
1. Complete the Health and Wellness Survey (be honest – we’re here to help!) and chart
documents at home.

have a chart at the Health Center the Chart Documents are not needed.
2. Bring your completed survey and chart documents to your first appointment. We will perform
a Complete Physical Examination. Blood work and referrals for mammogram will be given.
3. Have ordered blood work drawn before your second appointment. Blood can be drawn at the
Health Center or laboratory of your choice. Usual screening blood work includes Cholesterol,
Kidney and Liver function, blood sugar, check for anemia and thyroid function. HIV and STD
screening will be provided if requested.
4. Your next appointment will review your survey responses, your blood work results and address
any problems. Working together we will compose your wellness goals and reasonable ways to
get you there.
5. Medications for tobacco cessation can be prescribed if requested, as well as any other
medications for treatment of health problems uncovered during the physical exam or blood
work if requested. Or, if you would like to follow up with your Primary Care Provider copies
can be sent to them for further evaluation and treatment.
6. Every 2 weeks you will return to review and discuss progress with your Wellness Plan. We will
act as your guide, partner, and cheerleader!

Most Private Insurance companies cover a
Full physical exam annually!
Cost of Program:
If you have Private Health Insurance: Your first and second appointment will be billed to your insurance
and your co-pay collected. No Charge for remainder of visits during the summer! Please note - if you
have had a full physical exam and screening blood work provided by your PCP within a year, these
services may not be covered. Please contact your insurance carrier for more information. If you would
like to join the program but do not wish to have a physical exam or blood work, one appointment can be
made to review your questionnaire and compose your Wellness goals. This appointment will be billed to
your insurance and co-pay collected, with no charge for remainder of visits during the summer. To get
full value of the program, recent screening blood work should be reviewed during your first
appointment.
If you have Student Health Insurance or no insurance: The first visit is $50.00 and second visit is
$25.00 (Does not include any requested blood work or screening tests.) An appointment can be made
for a women’s health exam which will be provided for an additional $35.00.
