Use the box below to provide any additional information you would like the Committee to know about this proposal.
Issues to consider addressing include: transferability to UW-Madison or other local 4-year campuses, evidence of student
demand for this offering, and why you feel you are particularly well-suited to develop and/or teach this course. If the
proposal is for an online, hybrid, or accelerated course, please indicate any formal training and/or past experience you
have teaching in this format. The School of Arts and Sciences requires instructors who wish to develop or teach a course
in online, hybrid or accelerated format to have successfully completed the corresponding training course through CETL,
or show evidence of equivalent training elsewhere via the CETL Alternative Format Prior Learning/Experience
Assessment.
Please be aware that the Dean of Arts and Sciences has final authority to approve payment for proposals
requesting funding. Payment will generally not be approved until after the proposed course has been taught for
the first time and a completed course portfolio has been submitted. If the proposal is for a course in an alternative
format (online, hybrid, or accelerated), the contents of the portfolio should be tailored to this format.
As confirmation that the faculty member’s department has discussed, approved, and ranked this proposal, the signature of
the Department Chair should be obtained prior to submitting the proposal to the A&S Curriculum Committee.
