1. Terms and Conditions of Service

The terms and conditions applicable to this post are those of all NHS Employees.

2. Superannuation

You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related

Pension Scheme or to take out a Personal Pension.

Employee’s contributions to the NHS Scheme amount to 5% to 13.3% of salary (depending on rate of

Pensionable Pay) and the employers’ contribution equates to 13.5% of salary. Employees in the NHS

Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of

2 ROLE PURPOSE & CONTEXT

The primary aim of this post are to:

 Design; implement, and evaluate a collaborative, stepped-care model to promote psychological wellbeing among people with coronary heart disease (CHD) and heart failure (HF).

This post is part of a multi-region project funded by the Scottish Government to try to establish how best to effectively and efficiently provide psychological care to people with CHD and HF. It will require a range of skills including the ability to:

Implement potentially complex systems of care in existing acute medical services

Provide a time-limited psychological assessment and intervention service

Develop appropriate sign-posting systems; care pathways; and protocols consistent with the project aims

Train; supervise, and support colleagues with little or no knowledge or experience of psychological models and intervention techniques, in their efforts to maximise health outcomes

Initiate service evaluation / audit / research to inform service provision and future redesign efforts locally and at national level

3 KEY ASPECTS OF POST

Clinical Responsibilities

1. To design and deliver a highly specialist psychological assessments of CHD and HF patients based upon the appropriate use, interpretation and integration of highly complex data from a variety of sources, including medical notes; colleagues in cardiac services; psychometric tests; standardised self-report measures, rating scales, direct and indirect observations and semi-structured and structured interview schedules with patients, family members and others involved in the patient’s care.

2. To analyse highly complex, multi-source information about the patient and evaluate all potential outcomes before deciding on a specific action.

3. To develop and implement plans for the specialist psychological treatment and/or management of a patient’s emotional and/or self-management problems, based upon an appropriate conceptual framework of these problems, and employing methods based upon evidence of efficacy, across the full range of care settings.

4. To be responsible for implementing a range of psychological interventions for individuals, carers, families and groups, across the course and progression of CHD and HF, adjusting and refining psychological formulations, as new information becomes available, drawing upon different explanatory models and maintaining a number of provisional hypotheses.

5. To evaluate and make decisions about the best treatment options and the best course of action, taking into account both theoretical and therapeutic models and highly complex factors, unique in each case, concerning historical and developmental processes that have shaped the individual, family or group.

6. To exercise autonomous professional responsibility for the assessment, treatment and discharge of CHD and HF patients whose problems are managed by psychologically based care plans.

7. To provide highly specialist psychological advice, guidance and consultation to other professionals contributing directly to patient care, who may at times hold conflicting opinions. This includes guidance on problem formulation/diagnosis and the establishment and maintenance of psychological care/self-management and other treatment plans. To demonstrate the highest level of interpersonal and communication skills in providing psychological expertise.

8. To contribute directly and indirectly to a psychologically based framework of understanding and care for the benefit of all CHD and HF patients, across all settings and agencies serving the care group. To facilitate the use of psychological models and approaches in the care of CHD and HF patients and their families in a range of care settings through the provision of advice and consultation and the dissemination of psychological research and theory.

9. To undertake risk assessment and risk management for individual patients and provide advice to other professions on psychological aspects of risk assessment and risk management.

10. To communicate, orally and in writing, in a skilled and sensitive manner, information concerning the assessment, formulation and treatment plans of patients under their care and to monitor progress during the course both uni and multi-disciplinary care.

11. To provide expertise, advice and support to facilitate the effective and appropriate provision of psychological care by staff involved in the provision of care to CHD and HF patients.

12. To be responsible for the provision of a psychology service to CHD and HF patients in

NHS Forth Valley.

Clinical Teaching & Training

1. To provide pre and post-qualification teaching for clinical, health and counselling psychologists, as appropriate.

Supervision

1. To provide supervision as part of an overall evidence-based training package aimed at facilitating an increase in psychological knowledge and skills in staff working with CHD and HF patients and their families. postgraduate training.

7. To verify the accuracy of others' travel expenses, if required.

8. To develop and use complex multi-media materials for presentation in public and professional settings.

2. To provide clinical placements for trainee clinical psychologists, ensuring that trainees acquire the necessary skills, competencies and experience to contribute effectively to good care and to contribute to the assessment and evaluation of such competencies.

2. To provide advice, consultation and training to staff working with CHD and HF patients across a range of care settings. This will likely be a substantial element of this post.

3. To contribute to the teaching of under-graduate medical students and post graduate medical staff on relevant topics, as appropriate.

4. To provide teaching for nursing, medicine, occupational therapy, physiotherapy and other

Allied Health Professional staff as appropriate.

5. To continue to gain relevant wider highly specialist experience in a Continued Professional

Development (CPD) program as agreed with the line manager.

6. To maintain and develop skills in the area of clinical supervision particularly in relation to

6. To undertake regular clinical supervision as required at all grades of clinical psychology staff to meet their professional guidelines as per the British Psychological Society Code of

Conduct and the requirements of the Health and Care Professions Council.

7. To identify and monitor the practice of those you supervise and to identify deficits in performance and conduct and, where necessary, to initiate the initial stages of grievance and discipline procedures by alerting relevant line managers.

8. To carry out appraisals, where necessary, for Assistant Practitioners and Trainee Clinical

Psychologists.

Management

1. To over see the daily planning; reporting; organisation and delivery of project in NHS

Forth Valley.

2. To operationalise Scottish Government policy and practice documents and relevant clinical guidelines.

3. To participate as a senior clinician in the development of a high quality, responsive and accessible service including advising both service and professional management on those aspects of the service where psychological and/or organisational matters need addressing.

4. To participate in CHD and HF team meetings; liaise with senior clinicians and other staff members in ways that facilitate the successful completion of the project aims.

5. To exercise appropriate responsibility for the systematic governance of psychological practise within the service/ team.

6. To participate as appropriate in staff recruitment, both in the short-listing process and as a member of interview panels for Assistant Practitioners, Trainees Clinical Psychologists and Applied Psychologists.

7. To exercise delegated responsibility for managing the psychological resources available to a team, whether in the form of additional qualified and unqualified graduate psychology staff or in the form of psychological materials employed in the assessment and treatment of patients.

Policy and Service Evaluation

1. To have delegated responsibility for specific aspects of the development, implementation, monitoring and evaluation of the service’s operational policies and services, through the deployment of professional skills in research, service evaluation and audit. This will include all aspects of the project aims.

2. To advise both the Scottish Government and NHS Forth Valley in their efforts to consider the planning of psychological aspects of CHD and HF services. To contribute to changes to practice and service policy/development as appropriate, and be aware of the impact of such change on the service, other professionals and other agencies.

3. To serve on, and where appropriate, represent the service on multi-agency committees concerned with the development of applied psychology services in Forth Valley, as agreed with the line manager.

4. To be involved in identifying and implementing service developments to improve the quality of care provided by the service.

Research & Service Evaluation

1. To take the psychology lead, as a senior clinician, in the evaluation, monitoring and development of the project and operational policies, through the deployment of professional skills in research, service evaluation and audit and ensuring incorporation of psychological frameworks for understanding and provision of high quality care, as required.

2. To utilise theory, evidence-based literature and research to support evidence-based practice in individual work and work with other team members and to apply highly developed research skills to all aspects of work in a routine basis.

3. To conduct appropriate research and provide research advice to other staff undertaking research, particularly in areas relevant to the post holder’s service responsibilities, as agreed with the line manager.

4. To manage the project on a daily basis, including delivering complex audit and service evaluation, with colleagues within the service, to help develop service provision.

5. To take responsibility for the development of effective means of auditing the project overall; individual work, and the quality; effectiveness and efficiency of the services provided.

6. To be involved in organising and supervising research and development work of any

Assistant or Graduate Psychologist and MSc trainees employed within the service. To provide clinical supervision of doctoral research of trainee clinical psychologists.

General Responsibilities

1. To contribute to the development and maintenance of the highest professional standards of practice, through active participation in internal and external CPD training and development programmes, in consultation with professional and service managers. This will include both within NHS Forth Valley and at a national level.

2. To contribute to the development and articulation of best practice in psychology across the service and elsewhere in Scotland, by continuing to develop the skills of a reflective scientist practitioner, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the field of applied psychology; CHD and related disciplines and areas.

3. To maintain the highest standards of clinical record-keeping including electronic data entry and recording, report writing and the responsible exercise of professional self-governance in accordance with professional codes of practice and local policies and procedures.

4. To collate and provide information from others (e.g. Assistant Practitioners, Trainee

Clinical Psychologists, and junior staff) in order to monitor and evaluate the service.

5. To participate in organisational advisory processes as appropriate to grade.

6. To maintain up to date knowledge of legislation, national and local policies and issues in relation to both the specific care group and mental health.

7. To maintain good current knowledge of all relevant statutory legislation and national professional standards and policies and procedures and training requirements, together with the implications of these as stipulated by The British Psychological Society, the

HCPC, and by NHS Forth Valley and to comply with these appropriately.

8. To maintain good current knowledge of psychological theories, models and interventions appropriate for the postholder's areas of responsibilities; to ensure the development and maintenance of the highest professional standards of practice via pursuit of Continuing

Professional Development activities according to BPS and HCPC recommendations and requirements; to maintain detailed, on-line records of CPD plans and activities and outcomes to satisfactory completion.

Post-graduate Msc in

Health Psychology recognised as providing eligibility for registration as a Practitioner

Psychologist with the

HCPC, plus additional

Diploma in Cognitive

Behavioural Therapy, recognised as providing eligibility for accreditation as a

C.B.T. therapist with the BABCP, or equivalent.

Further post-doctoral training, research and study across a range of areas, including advanced clinical skills over a substantial period, which also includes work at specialist level.

It is a requirement of these posts to provide

EXPERIENCE clinical placements and supervision to trainee applied psychologists undertaking a

Doctorate in applied psychology. To enable the post-holder to do so they must be eligible for the relevant training which meets University accreditation criteria.

Eligibility for this requires postdoctoral clinical experience.

Registration with the

HCPC as an applied psychologist.

Training in clinical practice supervision

Significant supervised applied psychology practice experience in NHS or equivalent by a suitably qualified applied psychologist.

Requires experience of working with a wide variety of client groups, across the whole life course and presenting with the full range of clinical severity across the full range of care settings including outpatient, community, primary care, inpatient and residential care settings. This includes skill in maintaining a high degree of professionalism in the face of highly emotive and distressing problems, verbal abuse and the threat of physical abuse.

Experience of exercising full clinical responsibility for clients’ psychological care and treatment, both as an independent practitioner and also within the context of a multidisciplinary care plan.

Experience of teaching, training and/or professional and clinical supervision.
