Job Description:
Specific & Responsibilities:
 Obtains and records initial assessment and establishes priorities according to
patient age, symptoms and psychosocial needs.
 Re-assesses patients systematically to identify progress and trends that require
intervention.
 Develops and implements a measurable individualized plan of care for the
patient within the time frame established by the Nursing Standards of Care.
 Makes comprehensive nursing decisions based on the interpretation of facts
and evaluations of patient outcome; modifies the plan of care based upon the
evaluations.
 Participates in patient/significant others health education and discharge
planning.
 Implements and documents timely interventions appropriate to patient needs.
 Interprets and implements physician’s orders: performs treatments according
to accepted standards and department policies; evaluates and documents
effectiveness of treatments.
 Documents patient responses to intervention with 100% completion rate to
ensure completeness of patient care.
 Consults, re-evaluates and supplements multidisciplinary plan of care each
shift. Enters either evaluation or resolution date on the plan each shift.
 Using principles of medication preparation and administration via eye drops,
ear drops, nasal medication, nebulizer, per orem, buccal, sublingual,
medication from a vial and ampule, medication with needleless system,
mixing medications, administering intradermal, subcutaneous, intramuscular
medications, administering in Z-tract, intermittent intravenous medication,
medications by nasogastric tube, rectal medication, vaginal and topical
medication.
 Promotes oxygenation through: performing chest physiotherapy to loosen,
drains and removes excessive secretions, applying a nasal cannula/facemask,
performing oral, nasopharyngeal suctioning, collecting obtaining pulse
oximetry.
 Managing fluids and nutrition through: managing intake and output which
helps control fluid balance, testing capillary blood glucose, performing
venipuncture for blood specimen for laboratory analysis, selecting a vein for





















intravenous therapy, preparing solutions, inserting a catheter/intravenous lock
for intravenous therapy, calculating flow rate, regulating an intravenous fluid,
changing intravenous tubing and dressing, managing blood transfusion,
maintaining and discontinuing a nasogastric tube.
Managing elimination through: collecting a midstream urine specimen using
aseptic technique, collecting a urine specimen from an indwelling catheter,
applying a condom catheter, performing a male and female catheterization,
caring a urinary catheter to decrease bacterial contamination of bladder,
removing an indwelling catheter, irrigating a bladder/catheter and assisting in
the administration of an enema.
Promoting activity and mobility through: proper positioning of the body,
performing range of motion exercises, caring for a cast.
Promotes rest and comfort through: administering heat and cold therapy,
administering a sitz bath, administering a tepid sponge bath.
Promotes wound healing in changing sterile and nonsterile dressing,
managing a pressure ulcer, irrigating and managing a wound drain.
Assists physicians with procedures in the Emergency Department.
Treats patients and their families with respect and dignity. Identifies and
addresses psychosocial, cultural, ethnic and religious/spiritual needs of
patients and their families. Functions as liaison between administration,
patients, physicians and other healthcare providers.
Interacts professionally with patient and family and involves patient and
family in the formation of the plan of care.
Proving a proper preparation of body of deceased client through postmortem
care
To provide safe nursing practice in the delivery of health care to patients in
labor before and after delivery of baby including surgical procedures/cases.
Develop/Cultivate the highest professional standard in Delivery process
through education.
Monitor fetus and mother during labor
Perform vaginal examination of ready-to deliver women
Assisted doctors during delivery process.
Provide post-delivery nursing care to mother and baby
Educated mothers on taking care of babies and on importance of breastfeeding
Sterilized medical equipment required in the delivery room
Immunization status for tetanus toxoid booster should be determined at the
initial prenatal visit.
Auscultation of fetal heart sounds starting at 20 weeks AOG is recommended
to every prenatal visit
Pregnant women with signs & symptoms of preterm labor like low, dull
backache, menstrual like cramps, abdominal cramping (may be associated w/
diarrhea) and four or more uterine contractions per should be educated.
Maintains confidentiality in matters related to patient, family and client
facility staff
Provides care in a non-judgmental, non-discriminatory manner that is sensitive
to the adult or elderly patient’s and family’s diversity, preserving their
autonomy, dignity and rights

Assists in the coordination of services with medical staff, nursing staff, other
departments as well as other providers. Responds rapidly and effectively
during emergency situations.
