6.4.1 ID BADGE POLICY 10/10 PURPOSE: Bellin College is committed to providing a secure and customer focused work place. As part of this goal, the College has established an identification badge system for all staff, faculty and students to readily identify authorized personnel and to provide access to campus facilities. I. II. III. Definitions a. ID Badge – The official identification for all employees, students and vendors. It also provides security access to specified doors. Requirements a. Employees, students and vendors are responsible for safeguarding their own ID badge. Lost or stolen ID badges should be reported immediately to Information Technology Services to be disabled. b. ID badges are the property of the College and are to be returned upon separation from the College. Supervisors or appropriate designees are expected to collect ID badge on the last day of employment. This applies to student employees. c. Upon graduation, student ID badges are returned to and collected by Student Services. d. Academics Office is responsible for collecting student ID badges of students leaving the College due to withdrawal or dismissal. e. All badges are to be returned to student services (room 111) for disposal. Issuance of ID Badge a. Employees i. New employees are required to obtain an ID badge within the first two days of employment. ii. Supervisors will submit employee badge request through SysAid one week prior to employee start date. iii. Official Bellin College photo will be taken during first two days of employment. Photo can be taken prior to employment if feasible. Supervisor will coordinate date and time with Student Services (room 111). b. Students i. Undergraduate 1. Undergraduate student photos should be taken during orientation process by Student Services personnel. 2. VP of Student Services will coordinate photos for students unable to attend orientation. 3. Individual submittal of photo for undergraduate students is not allowed. ii. Graduate 1. MSN students should make every attempt to have their photo taken by Student Services personnel on campus. 2. MSN students unable to visit campus may submit photos for their official Bellin College ID badge. Photos must meet the following requirements. a. Photo should be taken with a solid color background. A blue background is preferred. b. A clear photo should be submitted. Distorted photos will not be accepted. c. Photo can be taken with personal digital camera providing it meets clarity requirements. Photo centers such as Walgreens may provide an alternative option. d. Photo should be submitted with the ability to view shoulder height and above. 3. Graduate student badge creation request should be submitted to Student Services by Graduate program administrative assistant. IV. Replacement ID Badge a. Fees associated with replacement badges are defined below. Lost, stolen or damaged badge $10 replacement fee Name change Credential change due to completion of educational degree or certification $10 replacement fee Faulty Card Employee position change or role modification resulting in title change No charge No charge No charge b. Replacement Procedures i. Request for replacement must be made through Student Services (Room 111). 1. Administration Office personnel will communicate badge change to IT services for deactivation of old badge and activation of new badge. ii. Fees must be paid prior to receiving replacement badge.
