FOOD VENDOR’S DUTIES AND OBLIGATIONS
 Remit booth fee (+ $5 license if required)
 Secure required permits
 Mail signed application, check, proof of insurance, health dept. permit, and fire permit (if required)
 Application deadline: June 1, 2012
 Comply with all federal, state and local laws
 Adequately staff booth during Festival hours, and inform all personnel of site rules and layout
 Provide all serving and display equipment, refrigeration, and storage. Provide appropriate connections for power, if needed
 Keep area surrounding booth clear of garbage, food products and/or boxes during Festival
 Completely clean booth area at festival closing utilizing recycling and refuse disposal ad designated sites
HOLD HARMLESS: As a condition of participation, the above named Food Business and/or individual(s) agree to indemnify and hold
harmless the City of Seattle and the Roosevelt Neighborhood Association, including its directors, committee members, volunteers,
and sponsors from any and all claims, actions, suits, losses, damages, liability of every type and nature, including (but not limited to)
all costs and legal expenses incurred by the Applicant or any other party, by reason of any activity arising under or in connection
with the Applicant’s omission, error, or negligence arising from any activity related to said participation. In the event that any
litigation is commenced by or against the Applicant his/her/its heirs or assigns, then the Applicant shall proceed and hold harmless
the Roosevelt Neighborhood Association, including its directors, committee members, volunteers, and sponsors and shall pay all
costs, expenses and reasonable attorney’s fees incurred or paid by the aforementioned entities in connection with such litigation.
The parties hereby agree to the terms & conditions of this agreement. This document takes effect when signed by both parties.

IMPORTANT: The City of
Seattle is requiring all food service businesses to find packaging alternatives to
throw-away food service containers, cups and other products in all food service businesses - restaurants, grocery
stores, delis, coffee shops and institutional cafeterias.
Effective July 1, 2010, all food service products designed for one-time-use must be replaced with one-time use
products that are either compostable or recyclable. Attached is a document outlining requirements and
