Job Description Job title: Nutrition Practitioner Location: Chelmsford (Travel around Essex essential) Hours: Full time - Note this role involves significant evening work and work patterns will vary from week to week Salary: £27,000 pa Background information MoreLife deliver non-surgical, clinically led weight management programmes for obese adults (BMI 40+) in Essex. Our specialist Tier 3 service uses a psychological approach to weight management and utilises a number of therapies to evoke behaviour change around diet and physical activity. As part of our team, you will work alongside the MoreLife Clinical Lead in Dietetics, the Programme Manager and the Contract Manager. Purpose of job To deliver and support elements of the adult behavioural change weight management programme and undertake motivational interviews for newly referred patients for the Tier 3 service. Support on clinical triaging of patients following initial assessment questionnaires and where appropriate carrying out dietary assessments in a 1:1 clinic setting. Responsibilities Motivational interviews for Tier 3 patients Take total responsibility for the Motivational Interviews (MI) of all Tier 3 patients. This includes the process aspects of the MI’s: o Identifying who requires an MI o Monitoring progress with contacting patients o Sending out pre-triage questionnaires on completion of the MI o Moving patients to the next stage of the referral management process as well as the actual MI. Delivery of the programme: Delivery of nutrition sessions in the initial phase of the programme Delivery of nutrition sessions in the maintenance phase of the programme Undertake nutrition review sessions for patients in the maintenance phase of the programme Delivery of the maintenance element of the MoreLife curriculum to groups of 15-20 patients Clinical Responsibility: Triage patients upon completion of pre-triage questionnaires working in line with the MoreLife triage process Lead on Groups MDT, ensuring patients not losing weight are offered appropriate additional support from the clinical team NUTR1 MoreLife (uk) Ltd, Churchwood Hall, Leeds Metropolitan University, Headingley, LS6 3QJ. EXPERIENCE SPECIAL ATTRIBUTES DESIRABLE HOW IDENTIFIED Application Experience in working with individuals on a one-to-one basis to support lifestyle change Experience undertaking group work Application/ Interview Interest in obesity The ability to organise and support activities that are appropriate to the target audience Interest in health promotion Confidence in supporting engaging lifestyle sessions Experience in undertaking motivational interviews Application/ Interview Facilitation/coaching skills Self-motivated Strong interpersonal skills and the ability to build trusting relationships with individuals A caring and empathetic attitude Ability to use initiative Be able to work under pressure and meet targets Strong organisational skills Excellent verbal and written communication skills Health and safety To maintain a positive attitude to health and safety in carrying out the duties of the post with special emphasis on the environment and safety of the individuals in your care.
