4-H Chef Creations Contest Entry Form

Participants will create a breakfast dish that is healthy, tasty, and creative, and may enter as an individual or team of two
(2). Teams have to be from the same age division to be eligible for competition. Teammate must submit their own entry
form to qualify. Dish must utilize at least three (3) food groups. Participants will be judged on nutrition, food safety, and
food preparation. Study the guide provided to help become familiar with these principles.
Be creative! Online and family recipes are acceptable. Participants must provide their own ingredients; cooking
equipment will be provided. Be prepared to speak to the judges about your dish. Specific contest information will be
provided in a confirmation email once the entry form is received.
