Student Health Examination and Related Requirements Students accepted into the nursing program are required to submit current forms of the following health status documentation: 1. Health/medical record form completed by a licensed physician or practitioner The form should include the status of any current mental or physical health problems, clearance for the student to enter the nursing program, and ability to perform essential functions. 2. Yearly negative TB skin test, initial two-step, or chest x-ray 3. Vaccinations a) Annual flu vaccination b) MMR (German measles, mumps, rubella) or titer c) Hepatitis B vaccination series or evidence of immunity (see Appendix F1-F3) 4. Current cardiopulmonary resuscitation (CPR) certification for healthcare providers (BLS/Infant/Child) Respiratory Therapy Program Student Requirements Health Policy Within eight weeks of acceptance into the respiratory therapy program, all students are required to submit a current (within one year) physical examination packet including the following documentation: 1. A completed SSCC Respiratory Therapy Program Health Form The student may not attend the clinical experience until the form is complete. 2. Evidence of receiving the first two injections of the Hepatitis B vaccination series The student must complete all three vaccines. 3. Documentation of a TB skin test, varicella immunity (immunization record or titer), and proof of tetanus and influenza vaccination 4. Current cardiopulmonary resuscitation (CPR) certification for healthcare providers (BLS/Infant/Child) 5. Written proof of current health insurance
