Medical Illustration Service The department is currently staffed by 4 NHS Medical Illustrators (3.2 FTE). Responsible for management of the Medical Illustration Service on a day-to-day basis to ensure continuation of the service at all times to the highest possible standard and to adhere to the Institute of Medical Illustrators Professional Code of Conduct, plus The University of Leeds and NHS Trust policies. Responsible for maintaining a balanced budget within an agreed budget, which is negotiated at the start of each financial year. Responsible directly to the University of Leeds and the NHS Trust on issues concerning governance, consent for photography, image quality, image security, confidentiality, copyright and storage of clinical images. To act as Project Manager for all developmental work within the service such as sourcing new projects and setting quality standards before handing to others to implement into day-to-day practice. To undertake clinical photography and other imaging for patient’s clinical records, education, research trials and quality improvement initiatives such as audit. Using highly specialized technical skills and following specialised training to be able to undertake a range of imaging modalities such as: Clinical photography Intra-oral photography Ultra-violet fluorescence Infrared photography 3D- and 4D (facial 3D motion capture) imaging Ensure the efficient provision and supervision of professional photographic services in all of the clinical disciplines related to Dentistry. All clinical images have the potential to be used in a court of law and may involve a requirement to attend court as an expert witness. Responsible for the management and administration of a networked clinical image database, providing electronic picture files to clinicians and trust staff for clinical records, training, publication and research. To ensure that consent procedures are adhered to and that photographic images are stored and distributed with appropriate security and confidentiality in line with Caldicott principles and Data Protection. Glossary of Terms Budget: Negotiated at the start of each financial year. Clinical Photography: Professional photography using a range of lighting techniques which relates to the recording of patients clinical conditions. This may be in theatres, wards, clinics or the clinical photographic studio. The images may be used for patients’ records, teaching, trials and research or publication. Clinical Trials Record System: The department is often involved in clinical trials at a national and international level. The patients who are photographed are anonymous and there is therefore a very careful management system which has to be managed and forwarded to the central research base. Consent, Security and Confidentiality of Images: The Manager is the lead expert in the management of clinical photography within the University of Leeds and the NHS Trust related to the Leeds Dental Institute. A Photographic Consent and Confidentiality Policy is reviewed annually by the Medical and Dental Illustration Management and aims to protect the patient’s rights, by setting standards for every member of staff in the department to follow. Consent Database: The consent of all photographic images from patients and public relation images are stored on a database, which requires managing. Public relation images have to be reviewed on an annual basis. Clinical images need to be checked for level of consent e.g. publication. Dental Image Database: An electronic database of electronic images relating to Dentistry. This database has to be managed and files stored and backed up safely. Digital Ikam Camera: A specialised camera and associated software which requires specialist training, which is used for recording images of the dentition in order to analyse the colour of teeth. General Photography: All photography which does not directly relate to patients. Examples of which are public relations photography, VIP visits, specimens/cultures, architectural, equipment, accidents/incidents and group photography. Graphic Design: The production of design and artwork, the majority of which is now on computers using PC computers with various software packages e.g. desktop publishing, design and 3D imaging packages. Examples of products created are patient information leaflets, scientific posters, teaching presentations, interactive CD’s, annual reports and newspapers etc. All information is reproducible in the printed form or using electronic media. HPC Educational Requirements for State Registration: The HPC accepted Clinical photographers for state registration in September 2004, to be authorised by Parliament, October 2005 with a proposed implementation in early 2006. This will require that all photographers will require degree level or equivalent to practice, on patients, and maintain a CPD portfolio reflecting ongoing training and awareness of current issues. They must demonstrate they have attended the specialist training courses associated with the clinical work they provide. Image Library Management: Managing the handling of all photographic images to include storage devices i.e. negatives or digital images (electronic), maintain confidentiality, log consent levels, image accessibility and control over reproduction/copyright issues. Intra Oral Photography: The recording of clinical condition inside the mouth using lip retractors and photographic dental mirrors e.g. orthodontic work, cleft palate, malignancies and skin conditions etc. Specially designed mirrors are accurately and safely inserted to reflect areas of the oral cavity which cannot normally be seen from the mouth opening. Management and Administration of a networked Clinical Image Database: This is the implementation of a system in preparation for PACS when the electronic patient records are fully operational. The image management system for digital images (iBase) is held in Medical and Dental Illustration. It provides an electronic picture file of patients for clinicians and Trust staff, for the use of clinical records, training, research and publication. It is managed in such a way that it limits or controls who has access to the images to protect confidentiality of the patient. Printing Services: Various different external print companies are used in the production of the design work (dependant on the quality and quantity output). Most of this work which requires external contractors is full colour production which is not achievable ‘in house’. This requires a working knowledge of print process to advise customers on the type of print required, quality, costs relating to quantity, time scales etc. and the ability to negotiate with the suppliers to the most economic production costs. Producing Business Cases: Justifying, by report, to Dean/Director or Finance Managers the reasons for new resources either financial or staff to change service or incorporate new technologies e.g. digital technology. Protocols: Professional guidelines for the standardisation of clinical recording which both national and local is: Professional code of conduct. Westminster scales of standardised photography – to guarantee comparability/repeatability of photographs. Patient photographic consent. Various department policies and procedures for all actions to be carried out on a daily basis. These written procedures are used to standardise work, required for training and for judging competencies. Service Development: Sourcing new areas of work such as expansion of current clinical demands. Developing graphics and computer related innovations with new software. Sourcing new customers and services: In order to sustain the level of work required to support the services and staff employed, a constant review of services and what is cost inefficient or cost productive needs analysing. Shortfalls due to demand falling e.g. PowerPoint slides or annual Trust savings need to be filled, to compensate, with new and innovative work. This is a constant challenge as the technology is changing so fast. Ultra Violet/Infra-Red Photography: These are two types of photography using special lighting and techniques to highlight conditions. Conventional light does not reveal what is beneath the skin surface – by using UV &IR, they reveal skin pigmentation which can be documented. VIDEO: Professional videography using a range of lighting techniques which relates to the recording of patients clinical conditions. This may be in theatres, wards, clinics or the clinical photographic studio.
