Q: How is eligibility for Hospital/Homebound determined?
A: The application process for Hospital/Homebound eligibility involves input in
three parts including: the parent/guardian, school/principal and the medical
doctor. A student’s eligibility and amount/duration of participation is driven by
direction from a licensed Florida physician/psychiatrist related to a physical or
psychiatric condition that will require at least 15 consecutive days of absence from
school.
Q: How do I enroll my child in Hospital/Homebound?
A: An application may be obtained by contacting the Hospital/Homebound office

Q: How long will it take to complete the application process?
A: Within 5 days of receipt of the completed three part application,
parent/guardian will be contacted to set up an eligibility meeting. If eligible, an
Individual Education Plan will be developed at the time of eligibility determination.
Q: How will I know if my application has been approved?
A: You will know your Hospital/Homebound Application has been approved when
you receive either a phone call or email to schedule an appointment from a
Program Representative that has been assigned to you.
Q: How long can my child participate in the Hospital/Homebound program?
A: The Hospital/Homebound program is designed to be a temporary educational
program to help students who are unable to attend school for medical/psychiatric
reasons. The amount of time is based on each student’s need and is determined
by the attending physician/psychiatrist.
Q: How will being a student in Hospital Homebound effect my child’s magnet status?
A: Your child may return to their magnet or choice school within a certain time
constraint. It is important that if any changes occur with your child the school
needs to be notified. This will help assure your status.
Q: Will the teacher come to our house to instruct my child?
A: Students in grades K-5 and/or students served through the Access Points
Curriculum are provided direct instruction in the home or hospital. Secondary
students access general education class via web instruction during regularly
scheduled class periods.
Q: How does my child access graduation activities/information/ceremony once they are a
graduating senior?
A: Hospital/Homebound will dismiss eligible seniors to their homeschool upon
meeting all graduation requirements (typically the end of May). It is the
student’s/parent’s responsibility to contact the graduation coordinator/counselor
at the student’s home school early in the school year to obtain activity schedules,
dates and deadlines to connect with optional senior activities and graduation
information.
