Project Narrative for Community Primary Prevention Implementation Grant Applications **Instructions** Types of Applications that Must Include This Document Priority health area applications Community Primary Prevention implementation applications This section contains specific instructions on describing in narrative form the activities you propose to accomplish with a Community Primary Prevention implementation grant. Eliminating Health Disparities Initiative grants will be awarded to community primary prevention implementation grantees to address a policy, system, or environmental barrier that will reduce the health disparity in one of the EDHI priority health areas In 10 pages or less, using12--point font, single or double space permitted, describe what you will do. a. What and who will you focus on? Briefly describe a policy, systems and environmental change(s) that will have a direct positive impact on the population(s) you propose working with to improve their health status. Include in this section: State the race or ethnicity of the population(s). The age group(s) and gender of the people you propose to work with. Information on local demographic of the population(s). What are you trying to change? b. State what policy, system or environmental areas you are planning to address. Explain why you chose this area. What you will accomplish? What are your proposed activities? Describe the strategies you are proposing to implement, why you believe this will lead to a policy, system, or environmental change. If this is an innovative approach to address policy, system, and/or environment change provide a description. Explain why you chose this activity and why you believe it will reduce the health disparity in one of the eight priority health areas of the racial/ethnic group(s) you have selected. Collaboration is required. Provide details on the role of your collaborative partners on your project. Define what the expected outcomes are for the proposed activities. The outcomes for a Community Primary Prevention grant must relate to a change in policy, system or environmental changes. A policy, system or environment change can include distribution of resources, social support and community relationships, safe housing; access to transportation, health care, and nutritious foods; and employment. Identify one or more measures that will indicate whether your activity had its desired outcome. Attach a work plan that reflects your innovative ideas in the proposed activities, using the work plan template included in the Appendix N. Identify staff, facilities/locations and the timeline you will employ in your proposed activities.
