November, 2014 FHC Community Education Foundation Scholarship Information and Application The FHC Community Education Foundation was created to foster and encourage members of the FHC extended family to seek post high school graduation educational opportunities, including four year college programs, two year, vocational, and shorter career specific programs and nontraditional (i.e. part time) studies. The “FHC extended family” includes FHC employees and their families worldwide, residents of surrounding Maine communities and families of our customers and vendors. This school year, the Foundation will give out 12 Scholarships in both the fall and spring semesters. Qualified applicants should be enrolled in a degree program at a recognized institution of higher education and be able to demonstrate their understanding of the value of education to their personal goals and to their local community. Each $1,000 award is earmarked for books and related educational expenses. Funds will be awarded directly to the student, ensuring that they will be tax free to the family and can be excluded from calculations involving any student scholarship awards. Scholarships will be based on academic standing, financial need, as well as content and quality of an essay. Parent, scholastic, and vendor references will be contacted when appropriate. Although these are grant awards, the Foundation expects recipients to give back to their communities in the future, either in a monetary capacity or by donating time so as to create a supportive environment in their resident communities. Each semester the foundation receives an increased number of applicants, causing the reviewers to be much more selective. Therefore, please ensure that your application contains all of the necessary documents listed above. Incomplete submissions will be immediately discarded and will NOT be considered. Spring 2015 Application deadline: December 15, 2014 for January, 2015 decision. Applications will be reviewed by a team appointed by the Foundation Board of Directors, which includes community members, local educational leaders, and FHC representatives. A transcript for the last year of study completed (either official or unofficial), including GPA, or a letter indicating why this cannot be supplied. 3. A short (< 300 words) essay on one of the following topics: Topic A: Discuss an idea you have for engaging more young people in community service on a regular basis. Topic B: Which community service activity has meant the most to you? How has this community service experience made you think differently about others or yourself? Topic C: What types of skills have you used or developed through your community service? How will this help you succeed in your education and life? 4. A short (< 200 words) description of the educational program and institution selected. 5. A short (< 200 words) summary of approximate total semester costs of the applicant’s program as well as a description of financial need, including a summary of how much both parent and student will be able to contribute. Please include a summary of other scholarships being received. 6. If the applicant has received an FHCCEF award for the previous semester, a letter (< 200 words), explaining how the scholarship funds were expended signed by the awardee. A student who has received a previous award will not be considered for future awards if this letter is not included. If you are a first time applicant, this document is not required.
