FUNCTIONAL BEHAVIORAL ASSESSMENT (FBA)
Date: 05/04/09
1. Description of Problem Behavior: (What is the inappropriate or unacceptable behavior to be targeted for
intervention? Be specific about who was involved and what happened before, during, and after the event.)
At home and in the school setting, when Student is presented with an academic or social/behavioral task that he does not
want to do, Student refuses to comply with requests, begins whining, complaining about physical ailments, and
demonstrates verbal and physical aggression. Problem behaviors happen on a consistent basis at varying times during
his school day. Student behavior cycles throughout the day and the intensity and frequency of inappropriate behaviors
can vary from low level non-compliance to high-level physical and verbal aggression.
2. Document the Pattern of Behavior (frequency, intensity, duration, environmental factors, and context in which the
behavior occurred)
Student resistance with going to school and participating with peers in a structured learning environment has gone on
most of his life, although the severity of the behavior has varied through the years. Since Student has attended
Discovery Elementary he has missed 10 1/2 days from December 1, 2008 through March 20, 2009 (January - 2 days,
February - 5 1/2 days, March - 3 days). During that same time period, there were many incidences documented by the
PBI staff of physical aggression (38 times), verbal aggression (22 times), and non-compliance (29 times).,
3. History (e.g., what circumstances make the behavior more likely to occur, medical or physical concerns, substance
abuse issues, stressful events in the student’s life)
Student has a history of academic and emotional difficulties throughout his school career. School absences and refusal to
attend school have been numerous. He has been diagnosed with ADHD, BiPolar - NOS, ODD, Anxiety Disorder, and Mood
Disorder. He is currently under the care of All Seasons Mental Health for medication management. In April of 2008,
Student was initially found eligible for special education services in the Boise School District under the category of Health
Impairment. He has a current Individualized Education Plan with goals in the area of improving social/emotional skills.
4. Effectiveness of Interventions (What have been used and were they effective?)
Several previous Functional Behavioral Assessments, and Behavior Plans have been mostly unsuccessful in managing the
non-compliance and aggressive behaviors. Plans have included strategies for preventing aggression and non-compliance
concerns, teaching of replacement skills, and steps to use when reacting to Student behavior.
5. Develop a Theory for the Function or Purpose of the Behavior
At this time, the team feels that Student refusals to comply with adult requests, and his verbal and physical aggressive
actions are demonstrated in order for him to escape/avoid tasks he does not want to do, and to gain power and control
over his environment, which at times may cause him significant anxiety.
