 Application Agreement
Acceptance to Program
I understand that space for participation in Leadership San Luis Obispo is limited, and submission of this application does
not guarantee acceptance into the program.
If accepted, I agree to submit a completed Assumption of Risk and Release of Liability Form. Forms will be distributed at the
retreat.
Attendance
I have reviewed the dates of the Leadership SLO retreat and day sessions and, if accepted into the program, agree to
devote the time and resources necessary to attend all sessions; arriving on time each day and being in attendance the full
day. Further, I will complete outside homework assignments as assigned.
I have authorization from the management of my company/organization for full participation.
I will be an active participant in the design and execution of the Class XXV Legacy Project.
I understand and acknowledge that if it becomes unavoidable to miss all or part of Leadership session:
 I will notify the Director immediately regarding my absence.
 I will be required to submit a letter regarding my absence to the Leadership SLO Board of Directors
 I may be required to complete a make-up assignment to present to my classmates as assigned by the Director.
 I understand that more than one absence can result in a discharge from the program; the status is determined by
the Leadership SLO Board on a case-by-case basis. There will be no tuition refund.

 Tuition Agreement
Your Leadership San Luis Obispo tuition includes:







2 nights accommodations at Wonder Valley Resort
5 meals at Wonder Valley Resort
A private coach on Tour Day
Coffee, tea, pastries and fruit at 9 Day Sessions
Lunch at 9 Day Sessions
Access and exposure to city and county leaders in industry, government and non profits
The right to claim kinship and affinity with over 800 Leadership SLO graduates working to make San Luis Obispo
County a better place to live for ourselves and for our children.
If accepted into Leadership SLO, I will remit tuition payment by December 1, 2015.
I understand that payment plans and scholarship assistance are available to me.
If I withdraw from the program, either voluntarily or involuntarily, I acknowledge and agree to pay any balance on tuition as
well as pay back any scholarship funds awarded to me.
I understand that tuition for Leadership San Luis Obispo is non-refundable.
I affirm that the previous commitment statements and the information submitted with this application are true and correct to
the best of my knowledge, and I agree to all of the terms of my application and potential participation in Leadership SLO.

Alternately, letters of recommendation may be mailed directly to the executive director at the address below. Only 2
letters of recommendation are accepted. In the event that more are submitted, the executive director will choose the 2
presented to the interview committee.
