FIFE PRIMARY CARE JOB DESCRIPTION POST: SPECIALTY DOCTOR IN COMMUNITY GENERAL ADULT PSYCHIATRY with special interest in acute care– WEST FIFE 1. INTRODUCTION The Adult Psychiatric Service in Fife is divided into three sectors – South Sector based in Kirkcaldy (population 147,380), North East Sector based in Cupar (population 74,180) and West Sector based around Dunfermline (population 126,840). The service offers a comprehensive range of care based in the community and in two District General Hospital units at Whyteman’s Brae Hospital, Kirkcaldy and Queen Margaret Hospital, Dunfermline and at Stratheden Hospital, Cupar with associated Day Hospitals. There are specialist services in Old Age Psychiatry, Rehabilitation, Intensive Psychiatric Care, Forensic Psychiatry and Addictions. The service is supported by strong Psychology, Community Nursing and Social Work services with whom there are excellent working relationships. Fifewide services in Learning Disabilities and Child & Adolescent Psychiatry are also provided by NHS Fife. The IPCU and specialist inpatient rehabilitation beds are based at Stratheden Hospital, while in the West Sector an assertive community treatment team (WFCOT) delivers care to people with severe and enduring mental illness. Mental Health Services are currently managed through Kirkcaldy and Levenmouth Community Healthcare Partnership (CHP), although this is likely to change as we move towards health and social care integration in 2015. 2. THE WEST SECTOR The area covers both urban and rural communities with the main town being Dunfermline. Phase 1 of Queen Margaret Hospital, Dunfermline was opened in May 1985 and has 30 acute beds for the under 65s, assessment, respite and continuing care for patients over the age of 65 suffering from mental illness and day hospital facilities for adult and old age psychiatry. The General Adult Mental Health Service in West Fife is split between four sector Consultants and each sector has dedicated Community Psychiatric Nursing and Occupational Therapy input. In addition the West Fife Community Outreach Team (WFCOT) cares for patients with severe and enduring mental illness who are difficult to engage. The Hillview Day Hospital provides one to one and group psychosocial interventions for patients with mild to moderate mental disorders. A Fife wide ECT service and Clozapine monitoring service is also provided at the Queen Margaret Hospital site. 3. THE POST Fife has committed to developing a comprehensive community based psychiatric service. The West Sector has priority to further develop the community general adult service in order to provide high quality, safe and effective care for people with all types of mental disorders. This is a new post that has been developed to build upon existing community based services. The post holder will work mainly under the direction and supervision of 2 consultants: Dr Beveridge and the other is a vacancy currently being adveritsed. You may have some small input to Dr McCallum’s outpatient clinic. The catchment area includes Dunfermline as well as the suburban towns and villages of Crossgates, High Valleyfield, Kincardine, Limekilns and Rosyth. One day per week you will participate in covering UCAT(unscheduled care assessment team based in Whytemans Brae Hsopital, Kirkcaldy) duty Dr rota as detailed below The deployment of the sessions will be agreed according to the postholder’s experience and the needs of the service and will include: 1. Taking responsibility for managing patients in the community with all types of mental disorder as well as contributing to the development of West Fife’s community general adult service. 2. Out-patient clinics – new and return, routine and urgent - as well as occasional home visits. Working with other team members on care planning and case management including using the Care Programme Approach. Diagnostic assessments, medication management and ensuring adequate monitoring of physical health will be priorities for the post holder. The use of the Mental Health (Care and Treatment) (Scotland) Act 2003 will also ideally be required. The post holder will liaise with the day hospital and inpatient units when patients under their care are admitted. 3. Cover as UCAT duty Dr one day each week providing urgent and emergency mental health assessments as part of the Unscheduled Care Team. Occasional Extra Day Duty sessions may be necessary to cover unforeseen absences or assistance with duty Dr care at QMH due to unforeseen circumstances 4. Liaising with colleagues from a range of disciplines from within Fife Primary Care including General Practitioners and Psychology as well as the Social Work Department, the Police and Voluntary Sector organisations. 5. Participation in clinical audit activities. 6. Being a member of the Fife Division of Psychiatry and playing an active part in its work. There are opportunities for teaching medical students and other health care professionals. In addition to the duties outlines above, you may occasionally be required to undertake other duties for limited periods. A job plan will be agreed on appointment. EMPLOYMENT DETAILS The post is available on a full time basis although job share may be an option and is currently based at Queen Margaret Hospital, Dunfermline. The postholder will be professionally responsible to the consultants with whom the clinician works and the Medical Director and managerially responsible to the Clinical Services Manager. NHS Fife has adopted a directorate structure. The General Manager, Mental Health Services, has responsibility for the management of the Mental Health Service. The postholder will be expected to liaise with colleagues, general practitioners and Social Work Department. In terms of remuneration, the basic package will be subject to negotiation and will include: Salary Travel Leave Study leave Office and secretarial support is provided through the Medical secretary based at Queen Margaret Hospital. The appointment is superannuable, unless you opt out of the NHS Superannuation Scheme. Staff who choose not to be in the scheme will automatically contribute to the State Earnings Related Pension Scheme (SERPS) unless they are excluded from doing so or have arranged a Personal Pension Plan. The holiday entitlement for Specialty Doctor posts is 5 weeks for the first two years’ service in the grade and thereafter 6 weeks per annum. Hours of work will be agreed through an individual job plan. The salary scale for the post will be £37,176 to £69,325. The starting salary and other terms and conditions are subject to the nationally agreed terms and conditions for SAS doctors. Employment is subject to three months' notice (or a longer period) on either side, but is subject to the provisions for appeal agreed between Fife Primary Care and the BMA Local Negotiating Committee.
