8/2014 Procedure for Receiving Student Transfers Including In State and Out of State Resource Teacher/ Case Manager SRS District Manager School Psychologists Resource Teacher/ Case Manager •Receives SpEd file, makes copies of MDT/Psych report, IEP and any other mental health/medical reports, etc. •Sends information to the School Psychologists at the McKinley Education Center. This should include a hard copy of the student’s records that may already be on the SRS system. MDT’s on SRS are often not as complete as the “hard copy” reports. •Contacts SRS District manager to request a transfer if the student is transferring from a school within the State of Nebraska. The County/District/School will be listed on the bottom of the student’s SpEd forms. Include student’s name as it appears on the forms in your email to the SRS District manager. •Requests the student's file in SRS from previous school district. •Activates the student's file in our district and assigns the SRS building manager, i.e., case manager. •Sends an e-mail notification to Building Manager, Resource teacher, School Psychologists, and SpEd Secretaries at McKinley. •Contacts SRS District Manager, if the student transferred from Out of State, as the student will need to be added to SRS. •Reviews the file and enters multidisciplinary verification statement on a DRAFT MDT and/or on a Determination Notice. •Notifies team if additional information is needed to determine eligibility under Nebraska Rule 51, i.e., consent for testing. •Sends all necessary paperwork to the Resource Teacher, including test score graphs. •Invites parent and team members to a meeting to discuss verification decision. Also, responsible for obtaining the signatures of all members on the DRAFT MDT. •RETURNS the SIGNED DRAFT MDT and/or the Signed DRAFT Determination Notice to School Psychologists at McKinley[MEC]. •If a Determination Notice is completed, please meet with the parents. If parent signature is not obtained, document reason on the Determination Notice, before sending it to the psychologist. •Convene the IEP Team to determine the student's goals and programming needs, when appropriate. A new IEP may be needed. •Sends the finalized MDT and/or Determination Notice to the Case Manager and to the Parents. School Psychologists
