Parent/Guardian Permission & Dance Guidelines
I give permission for my student (named above) to attend the Catholic School Dance.
By signing this agreement,
I understand that I will be asked to pick up said student, upon notification of any disruptive
behavior.
I am also releasing dance organizers and chaperones of any and all responsibility for my
child, including personal injury or damage to belongings that may be incurred due to any
unforeseen incidents.
Any financial liability that may be incurred by the student is the sole responsibility of the
authorizing parent or guardian and not the responsibility or liability of the dance organizers and
chaperones.
I accept responsibility to ensure that my child is dressed appropriately
- No exposed shoulders or midriffs
- No short skirts or short shorts
- No hanging trousers (below waist)
- No jeans
- No heelie shoes
- No hats or hoodies
NO Cell phones and electronic devices will be allowed at the dances and absolutely no
pictures are permitted. If you feel that your child needs to have one, it can be left with your
child’s school sign-in person.
No gum is allowed. Inappropriate dancing will also not be tolerated. I will bring my child directly to
the dance and check–in with an adult, and pick up my child from the dance and check out with an
adult, promptly at 10:00 pm.
