Staff/Teacher Appreciation Program (STAP) Gifts
Once all donations are collected, the PTO will purchase Middleton Chamber of Commerce gift certificates for the
teacher(s) indicated. If you have any questions please contact the PTO at questions@elmlawnpto.com
*STAP is an optional program and should not be viewed as mandatory.*
If you’d like to contribute to STAP, please fill out the form below and send along with a check made payable to
"Elm Lawn PTO" (indicate “STAP” on the memo line).
You may send the form and payment to either option below:
1. To school with your child (please make sure it is in a sealed envelope marked "Elm Lawn PTO STAP” with
your child's name, teacher's name and grade.)
a. Donations may be left in the office or given to Grade Level Parents (PTO/Grade liaisons), Classroom
Parents, or teachers, but must be directed to the PTO. -or2. Mail check & the completed form (below) to: Jennifer Stewart, 6329 Hubbard Ave., Middleton, WI 53562
