A flexible and motivated individual is required to lead the recently redesigned Chest Pain
Service Specialist Nursing Team across in-patient and out-patient services within NHS
Tayside.
Excellent communication and interpersonal skills are essential as are the ability to lead and
work as part of a team. You will be accountable for delivering on service provision, redesign
and improvement.
Applicants should have extensive post–registration experience and demonstrate expert
clinical and professional practice within a Cardiology setting.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register

1. JOB DETAILS
Job Title: Lead Nurse / Senior Specialist Nurse - Chest Pain

2. JOB PURPOSE
Summary
The post holder will have the responsibility for the planning, delivery and evaluation
of Chest Pain Specialist Nursing Services for in-patients and out-patients within NHS
Tayside and will provide clinical leadership to health and partnership organisations
to:






Ensure safe and effective clinical practice leading the development and
delivery of specialised care programmes for patients predominantly with chest
pain, other cardiological conditions and symptoms suggestive of conditions
e.g. Syncope
Enhance the patients experience of care based on assessment of their needs
Manage and develop the performance of the team
Contribute to the delivery of the organisations objectives
Designing and implementing specialist care pathways for chest pain patients
Enable and promote patients’ knowledge of their condition, self management
and independence
This aims of this service are to:
1. To ensure prompt and timely assessment, risk stratification, clinical
management and onward transfer of chest pain patients to an appropriate
environment for on-going management
2. To provide appropriate investigation and review for both in- patient and outpatient e.g. ETT; tilt testing; rapid access chest pain clinic
3. To facilitate the discharge of patients within Chest pain service who do not
require on going management

4. SCOPE AND RANGE
NHS Tayside covers some 7,558 square kilometres with a population of
approximately 394,000. The Chest Pain Specialist Nursing Service provides care
over NHS Tayside including acute and community hospital sites.
In collaboration with the multi-disciplinary team, the senior specialist nurse is
responsible for explaining the diagnosis, and co-ordinating treatment and
management of patients with chest pain, other cardiological conditions and
symptoms suggestive of conditions e.g. Syncope.
The senior specialist nurse has responsibility for:
 Ensuring that standards of practice are set, maintained, reviewed and
continuously developed and that staff deliver safe, effective, patient-centred,
efficient, timely and equitable care within their scope of practice as part of the
healthcare team
 Contributing to the management of the department budget by ensuring
effective use of physical, human and financial resources e.g. the nursing
resource and supplies including pharmaceutical supplies and equipment
 The professional development and line management of specialist nurses and
support staff working within the Chest Pain Service
 Creating an environment in which effective learning can take place
 Co-ordinating the involvement of the multidisciplinary team within designated
areas covered by the Chest Pain Service
 Leading the team in the management and co-ordination of the patient’s
journey from point of referral through to discharge.

 A variety of clinical settings including Wards, Clinics within acute and
community care settings across Tayside
5. MAIN DUTIES/RESPONSIBILITIES
Within the governance framework of the organisation (including clinical, staff and
financial governance) the post holder will focus and lead on the following key result
areas:
1. TO ENSURE SAFE & EFFECTIVE CLINICAL PRACTICE
1.1 Clinical Leadership and Teamwork
As a clinical specialist, promote teamwork within a multi-professional environment,
demonstrating critical analysis and decision making skills, leading the delivery of a
clinically excellent, high quality chest pain specialist nurse service, influencing and
facilitating change within the department and, where appropriate and in line with the
organisation’s objectives
1.2 Evidence Based Clinically Effective Practice
Act as a change agent leading the development of clinically effective practice through
the effective utilisation and integration of evidence; setting, implementing and
monitoring evidence based policies, procedures and protocols.
Responsible for explaining the diagnosis, and co-ordinating the treatment and
management of patients with Chest pain and other cardiological conditions as inpatients, out-patients; taking action to ensure appropriate clinical management of the
patient in accordance with local protocols.
1.3 Continuous Quality Improvement
Ensure a culture of continuous quality improvement through the use of audit, patient
feedback and reflection on practice by self and other members of the team.
Participate in local and national clinical trials and research programmes, informing
and recruiting patients.
Work in collaboration to initiate relevant nursing research within the service.
Lead the team in their responsibilities for data entry and storage of data using
computer based systems.
Produce reports for projects/audit/information related to aspects of work as
appropriate
Responsible for integration of learning into service improvement
1.4 Patient Safety
Responsible for promoting a safe and clean environment for staff, patients and
visitors by ensuring compliance with legislation, policies and protocols e.g. health and
safety, healthcare associated infection, risk management and critical incident
reporting and analysis, assessing and managing actual and potential risks to health
and well-being.
Ensures high standard of record keeping in accordance with Nursing & Midwifery
Council, national legislation and local standards, facilitating effective communication
with multi-professional team regarding patient care.
1.5 Patient Care
Ensure care needs are assessed, programmes of care are developed, implemented
and evaluated in partnership with partners/carers/clients/relatives for patients whose
needs may be predictable or unpredictable to ensure delivery of a high standard of
care. This will include advanced clinical assessment particular to Cardiology, e.g.
chest auscultation.
Prescribe and/or make recommendations within the multi-disciplinary team and within
primary care about patients’ medications particular to Cardiology, working within the
scope of their own professional practice, adhering to the NMC Standards for
Medicines management and the NHS Tayside Safe and Secure Handling of
Medicines Manual.
The post holder will provide appropriate education and psychological support to
patients and their families in response to individual need.
Contribute to health promotion activity raising the awareness of patients referred to
the service regarding their health and wellbeing and the actions that can be taken to
address issues
Demonstrate and role model advanced clinical decision making in relation to patient
care
2. TO ENHANCE THE PATIENT’S EXPEREINCE
2.1 Coordination of the Patient Journey
Responsible for ensuring the planning and co-ordination of the patient journey
including the smooth transition of patients to other settings, promoting effective
discharge and communication with interdisciplinary, interagency teams as required.
2.2 Clinical Expertise
Demonstrate and act as a role model, ensuring co-ordination of nursing interventions,
influencing clinical decisions and monitoring the quality of patient care provided
through using expert clinical knowledge relevant to Cardiology, underpinned by
theory and experience.
2.3 Promote a Culture of Person Centred Care
Within a multidisciplinary team environment, develop a culture of person-centred care
within the service, communicating regularly with patients, relatives and/or carers;
promoting a caring environment where equality and diversity issues are respected
and patients are enabled to be partners in their care.
Identifies opportunities to develop care and services by ensuring that there are
effective systems in place to ascertain patient and carer experience/feedback and
complaints are managed in line with organisational policy including the dissemination
of learning points.

Contribute to the development and delivery of educational sessions within the pre
and post registration curriculum
Contribute to the teaching of the multi-professional/multi agency groups
Support the career pathway of individual team members by ensuring all staff have
individual performance reviews and personal development plans and they maintain
an up-to-date knowledge of current clinical and professional issues and legalities
Responsible for ensuring that all team members have the appropriate knowledge and
skills to work within the Enhanced Chest pain Role Development Framework.
3.3 Managing the Practice Setting
Manage the practice setting, ensuring effective use of resources and workforce
planning by helping to monitor workload and maintaining appropriate staffing levels
taking account of role and competence of staff when delegating work, contributing to
the management of the budget.
Manage the nursing team by ensuring compliance by self and others with
professional standards including (NMC Code of Professional Conduct), legislation,
national and organisational policies, leading recruitment and selection, attendance
management, ensuring grievance and disciplinary matters within the service are
identified, actioned and reported to the appropriate manager

Works with health care teams to suggest and improve the services provided to the

Contribute to the avoidance of unnecessary hospital admissions through improved
patient self management and enhanced symptom control.
6. EQUIPMENT & MACHINERY
The Senior Specialist Nurse is expected to have and support others to have the
knowledge and skills necessary to use all equipment safely in the area The post
holder will be required to manage the use of the following equipment for the reasons
stated and is responsible for ensuring that systems/policies/procedures are
communicated to staff to ensure safe use, maintenance and storage of equipment in
the area:





Occasional use of moving and handling equipment used within the service
considering patients who are unpredictable, i.e. due to their diagnosis or
condition.
Medical and Technical specifically ETT and Tilt Testing Equipment for
investigation of conditions, equipment to record vital signs and administer
treatments including near patient testing to monitor physiological status as
required- frequent
IT equipment including local and national systems to read, analyse, record
and transmit patient and staff information within the boundaries of local and
national policies and legislation

7. DECISIONS & JUDGEMENTS
The Senior Specialist Nurse

Will be responsible to the Head of Nursing (Urgent Care) in respect of
guidance and professional management, work review and formal appraisal of
performance.

Will have continuing responsibility for setting and monitoring standards and
quality of clinical practice and ongoing supervision of the junior nursing team.

Has responsibility for supporting the nursing team to reflect upon and review
their decisions in relation to assessing, monitoring, evaluating and interpreting
patients’ condition and effectiveness of their care programmes.

Is also accountable for decisions relating to the management of physical,
human and financial resources within the service, including recruitment and
workforce planning.

Is responsible for making decisions and clinical judgements in relation to care
management underpinned by specialist knowledge and expertise.

Has responsibility for explaining the diagnosis, and co-ordinating the
treatment and management of patients with chest pain and other (suspected)
cardiological conditions managed within the service. This may include the
prescribing of medication.
8. COMMUNICATION & RELATIONSHIPS
Continuous responsibility for establishing systems and standards of communication
for routine, highly complex, highly sensitive and highly contentious and stressful
matters with a wide range of health and social care workers, patients, families, other
relevant departments/agencies using a wide range of media, using advanced
communication skills demonstrating sensitivity and empathy. to achieve agreement
or cooperation when e barriers exist.
Establish and maintain relationships based on mutual respect communicating on a
regular basis with the patient/relatives/multi-disciplinary team and external agencies
in the provision of care and services. Ensures appropriate systems are developed
and operationalised to facilitate dissemination of information up, down and across the
organisation.
Provide and report formal, informal support and counselling to team members.
Responsibility For Records Management
All records created in the course of the business of NHS Tayside are corporate
records and are public records under the terms of the Public Records (Scotland) Act
1937. This includes email messages and other electronic records. It is your
responsibility to ensure that you keep appropriate records of your work in NHS
Tayside and manage those records in keeping with the NHS Tayside Records
Management Policy and with any guidance produced by NHS Tayside specific to
your employment.
9. DEMANDS OF THE JOB (Physical, Mental, Emotional)
Physical Skills (frequent)
The post holder will require a range of physical skills relevant to clinical area
examples are use of diagnostic equipment, drug administration, clinical history taking
and assisting medical staff with invasive procedures.
Physical Demands (occasional)
The physical demands include patient movement with use of mechanical aids;
standing/walking for the majority of the shift; frequent restrictive movements to treat
patients e.g. stooping to perform specific interventions; community visits across
Tayside.
Mental Demands (frequent)
The mental demands include concentration required when checking
documents/patients notes and calculating drug doses as required, whilst subject to
frequent interruptions form patients/team members; concentration required when
observing patients behaviours/psychological status; balancing the competing
demands of the role while remaining available to staff, patients, families and/or
carers; maintaining high level and consistent professional behaviour in unpredictable
and stressful situations; meeting the needs of all stakeholders with finite resources;
keeping abreast of national and local policy and evidence based practice ,
interpreting applicability and adapting for local implementation; monitoring quality and
financial data developing action plans for improvement; continuously motivate,
enthuse and maintain morale of staff within an ever changing environment; working
as clinical specialist within multi-professional team; teaching and presenting to staff
and patient groups with differing knowledge levels.

Communicating with distressed/anxious/worried patients/ relatives/staff; caring for
patients with progressive illness or end of life; caring for and/or communicating bad
news to patients/relatives/staff; supporting patients with altered body image; dealing

process; meet deadlines in the face of conflicting demands.
Working Conditions (frequent)
Working conditions include for example exposure to body fluids; exposure to verbal
aggression; frequent hand washing; travel across Tayside, working within patients’
homes and out-patient clinics.

11. QUALIFICATIONS AND/OR EXPERIENCE SPECIFIED FOR THE POST








First level nursing or midwifery qualification with first degree or other
qualification equivalent to level 9 of Scottish Credit and Qualifications
Framework (SCQF)
Hold or willing to work to a recognised Master’s degree level 11 of Scottish
Credit and Qualifications Framework (SCQF) specialised qualification
relevant to specific post.
Appropriate part of NMC Register relevant to area
Post holder must have a minimum of 5 years post registration experience
within cardiology and/or demonstrate expert clinical and professional practice
developed through experience and theoretical knowledge
Evidence of post basic qualification in Cardiology
Have expert knowledge of the relevant anatomy physiology, disease process
and treatment options acting as an expert resource for Cardiology
Demonstrate the ability to accept and manage direct referrals, working
autonomously to assess, plan deliver and evaluate care.
Able to independently prescribe or be willing to undertake non-medical
prescribing course




Ability to maintain professional and personal credibility across all staff groups
demonstrating knowledge of and ability to implement current clinical policies
and guidelines
Ability to lead and manage teams, lead practice and continuous professional
development, working effectively as part of a multi-professional/multi-agency
team
Personal motivation and enthusiasm for the development of nursing, patient

The terms and conditions of service for this post are those
determined by the NHS Staff Council.
The current salary scale for the post is £30,764 to £40,558 per
annum (pro rata for part-time staff). Placing on the scale on
appointment is normally at the minimum but may be higher
subject to verification of previous relevant service.
Salary is paid monthly by Bank Credit Transfer.
The hours of the post are 37.5 per week.
Start and finish times will be determined by the needs of the
service.
Membership of the NHS Superannuation Scheme is not
compulsory but is open to all staff between the ages of 16 and
70 (65 in some instances).
The contributions paid are a percentage of superannuable pay,
which is essentially basic pay excluding, for example, overtime
or travelling expenses.
Contribution rates with effect from 01.04.13 are as follows:

Contributions are subject to tax relief and reduced National
Insurance contributions. NHS Tayside also makes a substantial
contribution towards scheme benefits – currently around 14% of
basic pay.
On appointment = 27 days (pro rata for part-time Staff) or 5.4
weeks per year.
After 5 years aggregated service = 29 days (pro rata for parttime staff) or 5.8 weeks per year
After 10 years aggregated service = 33 days (pro rata for parttime staff) or 6.6 weeks per year
Public Holidays = 8 days (pro rata for part-time staff) or 1.6
weeks per year
