To work within Community Nursing Team to provide care/Health promotion to a diverse client group some with complex health care needs, to ensure the highest standard of care is given which complies with current clinical guidelines and legislation. MAIN DUTIES/RESPONSIBILITIES Under the direction of the PHN/SHN, participate in the implementation of planned programmes of advice and support to children ,young people targeting individuals, families and vulnerable adults in the community and schools. Participate in the ongoing assessment, development implementation and monitoring of individualised programmes of health care ensuring that the highest standard of service and care is delivered in a consistent and safe manner, within the home clinical /educational setting. Undertake a range of clinical duties/nursing procedures ensuring all policies are adhered to. Initiate, establish and maintain good working relationships with patients, cares and members of the Primary Health Care Team to support multidisciplinary working. Establish and maintain effective communications and relationships/partnerships with statutory and voluntary agencies, which promotes collaborative working and effective co-ordination of services for individuals and groups. In collaboration with the Community Nursing team facilitate health promotion activities in line with the Health Improvement Agenda, ensuring inclusion of all relevant groups, collaborating with Primary Health Care teams, other professionals and organisations both statutory and voluntary. Identify and provide, in collaboration with the Public Health Nurse/SHN, ongoing support to families with complex problems eg mental ill health, learning disability, who may be unable to access other support services. Teaching, advising and providing support to patients and/or carers, throughout treatment, illness and rehabilitation. Organise and/or take part in immunisation providing information for audit as necessary. To contribute to the prevention, management and control of communicable diseases. To provide primary, secondary and tertiary preventive programmes with support from PHN/SHN within area of responsibility and for the wider community responding to local health needs to maximise health improvement eg smoking cessation, physical activity, weight control, . Work with clients to recognise and assess the social, economic, psychological and environmental factors which impinge on health and well being, and in collaboration with the PHN/SHN, set appropriate interventions with the individual, family and community eg nutrition , physical activity and social inclusion. Ensure Child Protection and Vulnerable Adults Policies and Procedures are followed, and undertake ongoing training, where appropriate, initiating action to identify and minimise risk. Maintain accurate and up to date clinical records/care plans in accordance with NHS Tayside and professional guidelines, GP practice requirements, legislation, best practice and patient confidentiality. To plan annual leave with other staff to ensure cover is provided during periods of absence. These skills are necessary to provide nursing care to children,young people and families. Support for patients and carers following receipt of bad news The postholder may have to convey or receive complex, sensitive and/or contentious information where there are significant barriers to acceptance which will need to be overcome using the highest level of interpersonal and communication skills, such as would be required when communicating in hostile, antagonistic or highly emotive atmospheres. Examples are in situations of aggression, the influence of substances or alcohol, mental health problems, learning disability, and for those for whom English is not the first language. General Conditions of Service POST REF NO: P/ER/63 JOB TITLE/GRADE: Registered Nurse, School Nursing, Band 5 LOCATION: North West Perthshire, Base Dunkeld Conditions of Service Remuneration Hours of Duty The terms and conditions of service for this post are those determined by the NHS Staff Council. The current salary scale for the post is £21,818 to £28,462 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer. The hours of the post are 24 per week. This is a fixed term/secondment post for a six month period during the testing of the new refocused school nurse model. Superannuation Start and finish times will be determined by the needs of the service.
