For Members of the MD Classes of 1995, 2000, 2005, and 2010
Nomination must be received by April 15, 2015
This award is presented to an alumna/alumnus who graduated from the Saint Louis University School of
Medicine within the last 20 years. Candidates have demonstrated excellence in medical practice or
research, or in their service to the community, or in their commitment to Saint Louis University.
GUIDELINES AND RESTRICTIONS:
 Any person may recommend an alumnus/alumna for this award.
 Nomination must include curriculum vitae and any additional, supportive information about the
nominee.
 The awardee must attend the Medical Alumni Reunion Dinner on Friday, October 16, 2015 in St.
Louis to accept the award.
