Abstract
This document describes the HiRadMat facility hazard inventory. The hazards have been identified for three
running modes of the facility: operation, maintenance and degraded (in case of malfunction).

1. GENERAL IDENTIFICATION
1.1 PROCESSES
The method used to identify the hazards is based on the breakdown of the HiRadMat
operational processes. They are represented on Figure 1, where each box represents a
process, the arrows from the left are inputs, the outputs go to the right and the
procedures enforceable are coming from down below.

To identify them, the processes are numbered and then divided into sub-processes,
which are also numbered after the parent process. The sub-processes are listed with
the corresponding numbers in Table 1 where the hazards can be investigated for each
task in more details.

This document allows a detailed identification of the work unit, tooling, material and
substances involved at each stage of the activity (column 2), and thus an accurate
identification of the corresponding hazards (column 3).
The potential (column 4) can be assessed on the reversible or non-reversible effects of
the hazard.
The safety rules or procedures in force ruling the hazard are listed column 5 and in
column 6 is seen if these rules entirely cover the work regarding the hazard. If it is not
covered, the column “Need for specialist” will be checked and a risk analysis will be
performed.

2.2 HAZARDS
Three functioning modes are considered: normal operation, maintenance and
degraded mode 1 . In the following tables, the hazards due to operation and
maintenance in HiRadMat are summarized.
The degraded modes will appear once the study is done.

Degraded mode : Partial or just ticking over operation of a process due to a malfunction. A particular organization
can continue operating while preparing for troubleshooting.
