Counselor Recommendation Packet
This packet must be completed and returned to the counseling office at least 2 weeks
prior to the deadline for submission of the recommendation. Please keep in mind that
counselors are not at school during holidays.

REGISTRAR
___ACT/SAT scores have been sent from the testing agencies
WAIVER
All college recommendations are confidential in nature. Therefore, we ask that you sign the
waiver below before asking your counselor to complete your college recommendation letter.
A parent/guardian signature is also required if you are not yet 18 years of age.
