Student Profile Packet for Recommendations
Complete this profile packet and turn it in to Mrs. Sherrill in the Counselors’ Office prior to requests for Letters of
Recommendation or requests to complete College Applications.
The Counselors’ require 15 school days to complete Letters of Recommendation, College Applications, and paperwork.

Authorization: I hereby authorize my counselor to write a letter of recommendation or complete
college applications which include confidential information such as GPA, class rank, grades, social
security number (if required) and other pertinent information. This request serves as an exception
to whatever CRHS privacy code I have selected and I waive the right to see the recommendation
submitted.
We ask that you sign below BEFORE asking your counselor to complete your college
recommendation letter. A parent/guardian signature is required if you are not yet 18 years of age.

◆Fill out all parts of these pages. Be sure that your name, birth date, address are complete and
legible.
◆Be specific in the information you use for each year. If you use an acronym for an
organization, be sure to explain it if it is not a common one. If you already have an activity
resume, you may use it instead of completing that section of the information packet.
◆When you respond to the items on the third page, please be specific. The more information
you give, the more informed and complete your counselor’s assessment of you can be.
Remember, the purpose of a counselor recommendation is to explain to the college what makes
you special!
