Role Profile: Project Manager – Health and Wellbeing Dementia Reading Champions Objectives To set up and implement the Dementia Reading Friends (DRF) Project in an effective and timely manner. To ensure the successful delivery of the DRF programme in specific settings To work closely with the Programme Manager and Volunteer Coordinator supporting the service provision to ensure clients needs are met and volunteers are suitably trained. To ensure agreed contract/grant outcomes and targets are achieved To contribute to the monitoring and evaluation of the project work. Organisation – Reporting to Programmes Manager Health & Wellbeing Duties and Responsibilities 1. Coordinate and implement the Dementia Reading Friends Project 2. Research and source relevant and appropriate literature (specifically poetry) and materials to support the project. 3. Liaise with and visit settings e.g. Care Homes, Day Centres, individual homes with a view to implementing the DRF sessions. 4. Work closely with the Volunteer Coordinator to ensure the project is appropriately supported with volunteers 5. Set up and implement a training programme for volunteers. 6. Set up and deliver information sessions for settings and organisations 7. To work with clients and settings to achieve their individual goals, and evaluate and record individual progress 8. Set up monitoring and evaluation processes. 9. Capture outcomes for DRF delivery in line with contractual requirements 10. Ensure effective and efficient communication to settings, clients and volunteers by working closely with project staff, partners and relevant agencies and any other agreed parties. Ensure procedure in relation to record keeping and monitoring is accurate and timely 13. Promote the DRF project, other Health and Wellbeing projects and any future appropriate schemes. 14. Work to ensure a high quality of service delivery to meet required standards. 15. To be aware of and adhere to Yorkshire Children’s Centre Safeguarding agenda 16. Ensure targets are met within the project as specified by the project plan 17. To provide regular and requested reports to the Programmes Manager as requested. 18. Complete any identified training required 19.
