2015-2016 School of Health and Human Sciences Contributions to Diversity and Inclusion
Award
The School of Health and Human Sciences Contributions to Diversity and Inclusion Award is
presented to the individual or individuals who make significant contributions to diversity, equity,
and inclusion in the School of Health and Human Sciences and in the health and human sciences
professions. Those contributions might take many forms, including: (1) increasing the presence
of under-represented populations (e.g., students, faculty) in the health and human sciences
professions; (2) conducting research that advances our understanding of the influences of social
disparities and inequity on the health, development, or well-being of diverse populations and
their communities; (3) developing model student success programs(e.g., retention, engagement)
focus on the needs of under-represented students; or (4) engaging and addressing the needs of
diverse populations facing social and health disparities in the Triad, state, and beyond.
The Awardee(s) will receive a $500 Award stipend and commemorative plaque. Faculty and
staff members are encouraged to nominate faculty colleagues they feel are deserving of this
award.
No past recipient of an award will be considered eligible for a second award in less than a fiveyear period.
Nominating Materials
Nominations materials should include:
 Brief CV highlighting elements of their contributions to diversity at UNCG and in the
health and human sciences professions.
 Description of Activity Meriting Award (no more than 1-2 pages). Include supporting
documents (e.g., relevant publications, support letters from students, faculty, community
members). Supporting documents may also include links to newspaper articles covering
the applicant’s activities.
 Signed letter of acknowledgment from candidate’s Chair (page 3, below)
