THE 1st ANNUAL DIVERSITY & INCLUSION AWARD Nomination Form Nomination Deadline: March 2nd, 2016 The Diversity and Inclusion Award recognizes the outstanding achievement of one undergraduate and one graduate student in the University of Guelph community, who support the University’s mission to create and maintain an inclusive environment for our increasingly diverse community. This award recognizes co-curricular initiatives1 that champion the values of human rights, accessibility, equity, and diversity and aim to diminish inequalities (within the context of race, class, gender, culture and other identities) on campus and to create pathways to resolving them indefinitely. The Diversity and Inclusion Award was established in 2014 (first presented in 2016) thanks to the generous annual donation of Jane Ngobia, Director, Diversity and Human Rights. The award is intended to promote and celebrate diversity on campus and to recognize the contributions of students who are working to create an inclusive environment. The Award honours one undergraduate and one graduate student or student group (composed of undergraduate or graduate students) who have contributed significantly to diversity and inclusion at the University of Guelph. This award consists of a certificate of honour and a permanent plaque displayed in the University Centre. The Annual Diversity and Inclusion Award – page 2 AWARD CRITERIA The recipients of the Diversity and Inclusion Award will have demonstrated a commitment to diminish and/or eradicate inequalities on campus through activities such as: • Modelling the values and behaviours of cooperation, fairness, inclusion, and respect for diversity, responsibility, and acceptance in the University of Guelph community. • Acting to identify and address inequities or barriers that may prevent fullparticipation of students from historically or currently under-represented groups in post-secondary education, which may include (but not limited to): o Students of Aboriginal or Indigenous ancestry o Racialized students o Students with disabilities o Students who are first or second-generation immigrants o First-generation university students (i.e. the first person in their family to attend post-secondary education) o Students from non-traditional families o LGTBQ+ students • Raising public awareness of the challenges and current issues relating to diversity and human rights. • Problem solving and taking action to develop solutions to inequalities on campus and within the community. • Demonstrating guidance and leadership in advancing knowledge, awareness, research and/or participation in diversity and human rights initiatives on campus. • Note that individuals employed or who volunteer in a capacity requiring expertise and/or responsibilities relating to diversity and inclusion are eligible for consideration. Nominators must clearly demonstrate how the nominee’s activities represent an effort that is above and beyond the day-to-day requirements of their role. REQUIRED SUPPORTING DOCUMENTATION Nominations will occur in the form of a completed application form and supporting documentation as follows: • A letter of nomination that includes details regarding the quantity and quality of the contributions made, and how diversity and inclusion was a focus for the nominee with a particular emphasis on the award criteria noted above. • A minimum of two letters of support from at least two of the following groups: o Student organization member(s) o Community organization member(s) o Faculty, staff, student, or others who support the nomination DOCUMENTATION CHECK LIST Indicate which supporting documentation you have provided with this nomination package.
