Advocate Health Care is committed to the personal and professional development of all associates. The Performance
Pathways process is intended to provide a structured process for managers and associates to share expectations about
performance and behavior and to partner in providing feedback on the development of short- and long-term learning goals.
This form should be used exclusively for zero-hour associates.
This form includes two sections that need to be completed for each associate by the manager. The third section is an overall
assessment of performance that is generated by the system from the manager’s ratings on the first two sections.
1) Review of job accountabilities, as defined on the associate’s job description,
2) Review of performance on Living Our Values through the Behaviors of Excellence,
3) Overall assessment of performance.
If applicable, annual operating unit or clinical competencies may be due at the time of this performance review, so ensure that
the associate has completed them and submit all documentation to the appropriate parties.
The Zero Hour Review Form must be completed and submitted online through Manager’s Desktop.
SECTION 1 – JOB ACCOUNTABILITES:
For each Job Accountability listed in the job description, consider the associate’s performance (i.e., quality, quantity,
timeliness, etc.) on objectives, work assignments, results, and/or projects. In the spaces below, please include behaviorallybased comments that focus on significant outcomes or achievements and areas for continued development. The job
accountabilities, as outlined in the job description, have been reviewed with the associate with the following areas noted:
