Testing Options









When requesting disability related accommodations associated with a diagnosed condition,
students must provide appropriate documentation of the significant functional limitations
associated with a diagnosed condition.
A diagnosis of a disorder, condition or syndrome in and of itself does not
automatically qualify a student for disability related accommodations.
There are instances where submitted documentation does not provide the information
required to substantiate significantly reduced functioning.
Documentation must meet WVU documentation criteria which can be found on the OAS
website at: http://accessibilityservices.wvu.edu/
Often an IEP or 504 plan from the public school system does not be provide adequate
diagnostic information needed to identify the functional limitation for which
accommodations were provided.
In order to obtain appropriate document that meets WVU documentation criteria a
student may obtain the documentation from any source they wish.
The attached resources are for the local Morgantown area, however, this list contains only
suggestions. There are other sources that you may want to pursue other than those listed.
Be aware that this information has been combined for convenience and may change at any
time at the discretion at the facility providing the assessment.

Documentation in the form of medical records, psychological reports, and/or information
submitted as documentation of a student’s disability and related functional limitations is
considered confidential by the Office of Accessibility Services (OAS). Upon submission to OAS by
the student, medical and psychological information is then regarded as educational information
and released to others relative to FERPA regulations. A student has the right to any information
that was submitted to OAS on his/her behalf. Specific diagnostic information is not released to
anyone inside or outside the University without the student's written permission. All information
contained in a student's OAS file is destroyed seven years after the student is graduated from
WVU.
