On-Line Access Patient Information Introduction Online Patient Access is a simple, secure and efficient system that allows you to book telephone triage appointments, order repeat prescriptions and view your medical records. This leaflet should give you all the information you need, but if you need more help please ask a member of our Practices Reception Team. Book You Next Appointment With Patient Access, you can view, book and cancel appointments at your convenience. Quick and easy to use Avoid busy telephone lines Access appointment booking out of Practice hours Electronic management of your appointments Appointments with a Clinician You can book a telephone triage consultation appointment with a clinician of your choice anytime of the day or night. Telephone triage consultation appointments are released from 12.01am via our automated appointment booking line. Therefore avoiding the need to telephone the Practice from 8am which is when the telephone lines are busiest. Nurse Appointments Due to the many difference types and lengths of nurse appointments available, we are unable to offer nurse appointments online at the moment. Please contact the Practice by telephone so that we can book you into the appropriate type of appointment. Cancelling Appointments If you are unable to attend for you appointment, we would appreciate you cancelling is at the earliest opportunity so that it can be offered to another patient. You can do this via the online system, by telephoning of by coming into the Practice. Inappropriate Use We monitor the use of this service, and hope you find it useful. However, if the Practice feel that any patients are misusing the service, we reserve the right to withdraw access to the service. Repeat Prescriptions With Patient Access you can; View a list of you current repeat medication list and easily select the ones you need to order. If you repeat medication is not listed, then you will need to contact the Practice. Include a message with your request (e.g. ask for your prescription to be sent to your local chemist) Track your order – Your request will be shown as ‘Accepted’ once it has been processed, or display an ‘R’ if your request has been rejected by your doctor. Please allow 72 hours for your prescription to be processed. Change of Details It is straight forward in Patient Access to let the surgery know of any change in your postal address, telephone number (mobile or landline) and E-Mail address. These will be actioned on a regular basis. It is important that we always have up to date contact details for you as we may need to get in touch with you urgently. doctor(s) as to which young people are competent to manage this system. Patient Access for Carers A carer may register for Patient Access with the written consent of the patient, lasting Power of Attorney or other authority confirmation. Access to Records You are also able to access a summary of your medical records. Patient Access for Children A parent/guardian who resides at the same address of children younger than 14 years my apply to register on behalf of their child for Patient Access. However, there may be young people who are capable of managing their healthcare and who will want their records to remain closed to their parents.
