We must receive 24 hrs advance notice for cancellation of any appointments or you are held responsible
for payment. Snow or severe weather late cancellation given under 24 hrs will be charged for half the session
fee. Please plan ahead for winter weather!

All payments are due when services are rendered. We accept check, cash and travelers checks. Currently
we do not take credit or debit cards. With no other option we charge the additional bank fees our clinic
incur to cover a convince charge of card use. A card number may be requested to hold your appointment
and will be charged in the case of late cancels and no shows.

If there are financial hardships, please discuss this beforehand to make arrangements. If arrangements
are made to pay in installments, we require predated checks. Any payments past Net 30 days of the
invoice or service will be billed a $20 late fee.

Returned checks will have a $20 fee for processing and bank charges.

Consult appointments by phone for out of town clients are done on a case by case basis at the
practitioners discretion and are not available in certain cases. Refills or formulas are not given without
consultation and re-checks. There is no "calling in and getting more."

Specifically, we are not available or responsible for being “on call” for frequent or detailed phone calls
and emails. Frequent questions by email or phone will require charges for consultation time. We will do
our best to get you an appointment at your earliest opportunity, but cannot guarantee to get you in
immediately. If you have a medical emergency call your licensed medical care provider. You will be
billed accordingly for frequent out of office contact based on the practitioner’s discretion. Please note:
Text Messages ARE NOT received; we do not communicate by text.

Separate appointments are made for Kunye Massage & External Therapies and Pulse / Urine
diagnostics. Unless the practitioner requests you to bring along your urine during an external treatment
appointment this is our policy. We are unavailable to check your pulse and urine without additional fees
when you come in for external therapies; both due to time constraints and for fairness of those who pay
for diagnostic appointments.

We respectfully ask you to be on time for your appointments. If you are late we may be unable to give
you the full 60, 90 or 120 minutes set aside for your appointment. If you have questions regarding your
next appointment or other issues, please cover that within your allotted appointment time. Otherwise we
may ask you to make an additional appointment to cover any additional concerns you have. If we take
additional time with the person in front of you, please have patience. We do our best to make that time
up to you just as is common in a western MD physician's office. However, we cannot always make up
lost time from your tardiness.

We have a no returns or refunds for any goods or services given policy.
These policies are briefly outlined in both the Preparations Form and Informed Consent.
By signing this policy form at the initial consult or massage, you agree to the clinic's policies and
procedures regarding appointments, scheduling, phone calls, texts and payments.
