Liability Waiver for The Southwest Rink at Park Tavern:
For consideration which Holder acknowledges, Holder expressly agrees:
Holder irrevocably grants to Parkside Mill Inc the right to use Holder’s image, likeness, voice, actions and/or statements in all forms and media including
composite or modified representations for all purposes, including advertising, trade, or any commercial purpose throughout the world and in perpetuity.
Holder hereby waives any and all claims that Holder may have for libel, defamation, invasion of privacy or right of publicity, infringement or copyright or
violation of any other right arising out of or relating to any utilization of Holder’s image as described herein. In other words, I know that my picture may be
taken at this event, and it may appear on the website or on promotional material.
Holder voluntarily agrees that Parkside Mill Inc may use the email address provided to inform holder of future events and promotions .Holder voluntarily
agrees that Parkside Mill Inc and all of their respective agents, officers, directors, owners and employees are expressly released from any and all claims
arising from the event including claims of negligence.
All Ticket Sales Are Final – No Money Refunded – No Exchanges. In the event of postponement of the event the ticket will be honored on the date the event
is held. Use of this ticket constitutes acceptance of these terms.
WARNING! HOLDER VOLUNTARILY ASSUMES ALL RISKS AND DANGER INCIDENTAL TO THE EVENT FOR WHICH THE TICKET IS ISSUED, WHETHER
OCCURRING PRIOR TO, DURING OR AFTER THE EVENT. HOLDER VOLUNTARILY AGREES THAT THE MANAGEMENT, FACILITY, PARTICIPANTS,
PARTICIPATING CLUBS, XORBIA, AND ALL OF THEIR RESPECTIVE AGENTS, OFFICERS, DIRECTORS, OWNERS AND EMPLOYEES ARE EXPRESSLY
RELEASED BY HOLDER FROM ANY CLAIMS ARISING FROM SUCH CAUSES. This ticket is not subject to any refund and shall bear no cash value, although
tickets purchased are transferable. If issued complimentary, this ticket shall not be exchangeable.
IN THE EVENT OF A CANCELLATION OR RESCHEDULING OF THE APPLICABLE EVENT, MANAGEMENT SHALL NOT BE REQUIRED TO ISSUE A REFUND
PROVIDED THAT YOU ARE GIVEN THE RIGHT, WITHIN TWELVE MONTHS OF THE DATE OF THE ORIGINAL EVENT, TO ATTEND A RESCHEDULED
PERFORMANCE OF THE SAME EVENT OR TO EXCHANGE THIS TICKET FOR A TICKET, COMPARABLE IN PRICE AND LOCATION, TO ANOTHER SIMILAR
EVENT AS DESIGNATED BY MANAGEMENT EXCEPT AS OTHERWISE PROVIDED BY LAW.

for seizure and cancellation without compensation. Tickets obtained from unauthorized sources may be lost, stolen or counterfeit, and if so are void. Holder
agrees by use of this ticket, not to transmit or aid in transmitting any description, account, picture, or reproduction of the game, performance, exhibition or
event for which this ticket is issued. Holder acknowledges that the event may be broadcast or otherwise publicized, and hereby grants permission to utilize
holder's image or likeness in connection with any live or recorded transmission or reproduction of such event.
In conformance with some local requirements or certain facility rules, alcoholic beverages, illegal drugs, controlled substances, bundles and containers of
any kind may not be brought into the premises. This ticket CAN be replaced if lost, stolen or destroyed, given an advanced notice of 48 hours before the
schedule start of an event. This ticket is not redeemable for cash. It is unlawful to reproduce this ticket. Ice skating is a dangerous sport that may result in
serious injury or death. We strongly recommend wearing protective gear including: helmet, elbow pads, knee pads, wrist pads, etc. Children under the age of
16 years should be supervised at all times by a parent or legal guardian. Reckless behavior on ice will not be tolerated and any persons engaging in such
behavior will be asked to leave without a refund.
