Dear Parents and Guardians of Grade Nine students,
The Munroe Junior High Grade Nine Farewell event will take place on Monday, June 29th, 2015. The
Farewell Ceremony will take place at the Holy Eucharist Parish Hall (across from Polson School) from
4:30pm to 5:30pm. Family and friends are invited to attend this ceremony. Awards and grade nine
certificates will be handed out at this time. After the ceremony, the grade 9 students and their teachers
will be celebrating with a dinner and dance at the hall.
The grade nine students should arrive at the Holy Eucharist Church Hall at 4pm sharp to prepare for
the ceremony. There will be an opportunity for pictures between 5:30pm and 6pm at the hall or on the
school grounds. Students will then be asked to enter the hall for the evening from 6pm to 10pm.
Students are expected to remain in the venue at all times. If a student chooses to leave the event early,
they will not be re-admitted to the venue. It is an air conditioned facility and staff must ensure that
students are supervised at all times. Students may be picked up from the Holy Eucharist Parish Hall
parking lot at 10pm. Outside guests are not permitted to attend this event.
The cost to attend the Farewell Dinner and Dance is $35 per student (cash or cheque payable to
Munroe Junior High School). There is no cost to participate in the Farewell Ceremony.
As this is a farewell celebration of the students’ experience at Munroe Junior High, please note that there
is no need to buy or rent expensive formal clothing. Students may dress in semi-formal attire. As this is a
school function, the school dress code is in effect. If your child has purchased an outfit that does not
follow school dress code (ex. Shoulder-less dress, spaghetti straps, muscle shirt) boleros, cardigans,
scarves and shirts should accompany them.
Please sign and return the form attached, along with $35, by Friday, June 12th, 2015 to allow your child
to attend the Farewell Dinner and Dance on June 29th, 2015.

procedures, to provide for thorough preparation and planning and implementation of out of
school activities undertaken by its employees, agents and officers.

in sports, recreational activities and other off-school site programs. These types of injuries may be

school/hospital to contact me, my signature on this form authorizes the board, through its employees,
agents, and officers, [to] secure such medical advice and services as they deem necessary for my child’s
health and safety, and that I shall be financially responsible for such advice and services.
I / We understand that the Rules and Regulations pertaining to this activity are designed for the safety
and protection of participants
I / We acknowledge my right to obtain as much information as I require about this program or activity
and associated risks and hazards, including information beyond that provided to me by the school.
I / We acknowledge that it is my responsibility to advise the school of any medical and/or health
concerns of my child that may affect his/her participation in the stated program or activity.
I / We understand that the choice to participate brings with the individual the ASSUMPTION OF RISK
which is part of those activities.
I / We understand and agree that this is a part of the school program. I/We also understand that as a
result of participating in this program that the participant is expected to follow the school procedures
and code of conduct and that any deviations from these may result in consequences from the school
administration.
I / We declare having read and understood the above INFORMED CONSENT AGREEMENT in its entirety
and hereby consent to participate being aware of all the foregoing.
