2.
Class of 2014 should attach pages 1, 2 and 12 of your State Degree
application. If you did not apply the application it is posted in the public
folder on the Crestview server.
3.
Attach a copy of your high school/or college transcript from the guidance
office/registrar.
4.
Class of 2014 and all renewal scholarship applicants should attach a resume’.
Must include college plans and major.

All scholarships are paid with the understanding that the money will be used for
tuition, fees, books, and equipment required for classes to attain a vocational diploma
or college degree from a postsecondary vocational/technical school or from a college
or university. Scholarship will be paid after the first quarter/semester with proof of
completion.
