*This checklist is to be used by the laboratory as preparations are made to close down the lab permanently or relocate within Wake Forest School of Medicine. Environmental Health and Safety will work with designated lab personnel to ensure that each item is completed.*

Chemicals

Shipping of chemicals; contact Laboratory Closeout Coordinator for price quotes and to make arrangements.

Segregate and label all “Hazardous Waste” chemicals for disposal.

Contact Stephen Phibbs (716-1241) or Matt Fields (716-1224) in Environmental

Health and Safety for disposal instructions.

Place glassware in a properly lined, sturdy box for disposal.

Clean fume hood with hospital grade disinfectant.

Biohazards (microorganisms and potentially infectious materials)

Arrange to have Biosafety Cabinet(s) decontaminated. It is the PI’s responsibility to have this completed before the lab closeout can be completed.

Contact Precision Air: (919) 212-1300 to schedule decontamination.

Disinfect incubators, centrifuges, ovens, refrigerators and other potentially contaminated equipment and surfaces.

Use appropriate solution: Pronto TB or hospital grade disinfectant.

Segregate materials for disposal or transfer.

Controlled Substances

Controlled substance permits are issued by the U.S. Drug Enforcement Agency (DEA) to individual researchers. There is no central record of permit holders.

Abandonment of a controlled substance is a violation of the DEA permit. DEA office:

919-733-1765. Permission to transfer ownership of a controlled substance must be approved by DEA.

Equipment and Lab Furniture

Bench surfaces

Remove absorbent paper, signs, notes, tape and labels; clean with surface disinfectant.

Clean and decontaminate any surfaces and equipment that was in contact with hazardous chemicals. Use appropriate solution: soap and water, Pronto TB or hospital grade disinfectant. Contact Pat Martin, Environmental Services: 716-4417 to obtain Pronto TB.

Laboratory Instrumentation

 Follow manufacturer’s instructions on shipping and moving.

Call A&D to advise of move and approximate pack and move date

If chemicals are being relocated from one building to another or from one floor to another, Laboratory Manager should arrange for EH&S Chemical group to help relocate the chemicals.

For chemicals, chemical and biological waste

Take cart, waste labels and assorted plastic bags to

PI one or two weeks before move date

Have PI/LM complete waste ticket

Once cart is full, have PI/LM call for pickup

Deliver a bottle of Pronto TB to clean and decontaminate any surfaces and equipment that was in contact with hazardous chemicals.
