Monday Tuesday Wednesday Thursday Friday 6.45am – 6.30pm 6.45am – 6.30pm 6.45am – 6.30pm 6.45am – 6.30pm 6.45am – 6.30pm DOCTORS & TRAINEES Since our last newsletter, we have said goodbye to Sr Mary Wilson Dr SAB Munshi will be leaving the practice on 31 August 2015 Dr Lisa Collins will be leaving the practice on 7 September 2015 Departing Trainees: Dr Barnaby Morgan, has recently left Dr Azeem Wahla, has recently left Dr Molly Leak, has started August 2015 Dr Divanka Wijendra, will be starting August 2015 Sr Paula Morrell (Practice Nurse) has recently joined the practice Maryam Jiwa (Pharmacist) is attached to the practice for the next 6 months DNA SCREENING The practice encourages patients to attend for national screening programmes eg breast screening, bowel cancer screening, Aortic Abdominal Aneurysm screening, cervical cancer screening. CHLAMYDIA SCREENING We offer confidential chlamydia screening to 16 to 24 years old patients, via a simple urine test, the results of which are sent directly to the patient. If interested see a doctor or practice nurse HEALTH CHECKS FOR 40 YEAR OLDS AND OVER: If you are 40 years or over and have no pre-existing significant medical illnesses, please contact the surgery to book an appointment with Sue, our Assistant Practitioner. These checks are used to help promote good health and also to screen you for problems such as high blood pressure, risk of heart disease/stroke and diabetes, so that these problems can be diagnosed early and treated more effectively ** STOP PRESS** Flu invitations will be sent to those who are at risk, from August 2015 HOLIDAY VACCINATIONS: If you are travelling abroad over the summer months please think about any holiday vaccinations/travel advice you may need. Ask at reception or complete on line a travel vaccination questionnaire. Please book your appointment 2 months before travel to ensure adequate vaccination courses completed. CORRECT CONTACT DETAILS: If you have changed your address, home telephone number, mobile number, or email address, please inform Reception. ON LINE SERVICES: We offer patients the opportunity to book, check and cancel GP appointments and order repeat medications 24 hours a day, 7 days a week. To register for this service, you will need a completed consent form (available at Reception),1 form of photographic identification (Passport or Driving Licence) or 2 forms of any other nonphotographic identification (eg utility bill and bank statement) to the practice. The Receptionist will then issue you with a unique username and password. You will be given a print-out of your username, password and the practice website address www.kearsleymedicalcentre.nhs.uk There is also an Online App available- ask at Reception for details. CARERS: If you are looking after a relative or a friend, then you are a Carer. In order that our GP practice can do all they can to help you, it is important that we are aware of your status as a Carer. Please collect a Carers Form from Reception, and then return your completed form to Reception. FRIENDS AND FAMILY: The Friends and Family Test is a patient feedback survey that enables the patient to inform us of their opinion with regard to service and treatment. It consists of just 2 questions. There are forms in the wall tray, next to the main entrance. Once you have completed the survey, please hand the completed form to the Reception desk. This survey can also be taken on line at www.kearsleymedicalcentre.nhs.uk PATIENT PARTICIPATION GROUP (PPG): The practice has an established Patient Participation Group. Interested?
