Dear
,
To properly evaluate the cosmetic enhancements that may be best for you, we recommend that you
take some time to study your face over the next few days. Make notations regarding the following:
1. What do you like?
2. What do you not like?
3. What changes would you like to see occur?
You may wish to look at Juvederm.ca/before-and-after/visualizer to consider the possibilities. As you
consider the questions above, it may be helpful to consider the different areas of the face:

Our Philosophy:
It is our position that the planning session is an important exercise to avoid the pitfalls of “too much”,
which occurs when one area of the face is emphasized over other areas. This is where the concept “Less
is More” is important to embrace. It is better to consider Botox and dermal fillers in smaller amounts in
all areas of concern with application, enhancements, and maintenance over time. This approach will
help keep the investment aspects of time and money in a better balance as you proportionally transition
to the look you will like. Periodic maintenance is important to avoid “starting over”. Of course, it is
helpful to consider cellular nutrition and skin care to improve the overall look and longevity.
Our planning session will then address the following:
1. What is your opinion regarding the areas of need? In other words, carefully evaluate what it is
you want or are trying to achieve.
2. Our assessment as to what can benefit you.
3. A coming together of 1 and 2 above
4. An estimate of initial or “start-up” costs as well as maintenance costs
5. An estimate of frequency of visits to avoid the “starting-over” challenges
When “timing” the selected procedures, it is important to understand that you more than likely will see
immediate results with the dermal fillers that will only improve at around two weeks, but the Botox
usually takes about two weeks before you begin to see results.
I trust that this format will be helpful for you to consider your “wish list”. We look forward to helping
you develop a plan with the “big picture” in mind. Thank you for your confidence.
Sincerely,
J. Michael Dierkes
